多囊卵巢综合征患者内分泌及代谢特征临床分析

摘　　要：目的探讨多囊卵巢综合征(PCOS)患者内分泌及代谢特征的改变。方法对肥胖型PCOS(A组)32例、非肥胖型PCOS(B组)28例、正常对照(C组)26例行口服葡萄糖耐量试验和胰岛素释放试验,并测定其血压、体重指数(BM I)、性激素、血脂,计算胰岛素敏感指数(ISI)。结果①与B、C组相比,A组收缩压、BM I、甘油三酯、低密度脂蛋白-胆固醇升高,高密度脂蛋白-胆固醇降低(P<0.05或P<0.01);②与C组相比,A、B组行睾酮(T)升高;与B组相比,A组T升高更明显;与A、C组相比,B组LH、LH/FSH升高(P<0.05或P<0.01);③与C组相比,A、B组空腹胰岛素升高、ISI降低;与B、C组相比,A组服糖后2h及3h胰岛素升高(P<0.05或P<0.01)。结论PCOS患者存在明显的内分泌及代谢紊乱,尤以肥胖型PCOS患者更严重,其远期并发症的防治应受到高度重视。Objective To investigate the endocrinal abnormalities and metabolic features of women with polycystic ovary syn- drome （PCOS） . Methods Thirty - two cascs of obese womcn（ group A） with PCOS,28 cases of non - obese women （ group B） with PCOS and 26 eases of control group （group C） were performed 75g oral glucose tolerance test and insulin releasing test. The blood pressure,body mass index （ BMI）, sex I lonnonc, blood lipid were also determined. Insulin sensitivity index （ ISI ） were computed. Results ①To compare with group B, C, the levels of systolic pressure, BMI, tfiglyceride,low density lipoprotein - cholesterol were significantly higher, but the levels of high density lipoprotein -cholesterol were significantly lower in group A （P 〈 0.05 or P 〈 0.01 ）. ②To compare with group C, the levels of testosterone were significantly higher in group A, B;the levels of testosterone were significantly higher in group A than in group B; To compare with group A,C,the levels of LH, LH/FSH were significantly higher in group B（ P 〈0.05 or P 〈0.01 ）. ③To compare with group C,the levels of fasting insulin were significantly higher and the levels of ISI were significantly lower in group A, B. To compar with gronp B, C, the levels of 2h postprandial insulin and 3h postprandial insulin were significantly higher（ P 〈 0.05 or P 〈 0.01 ）. Conclusion Those PCOS women have severe dysfunction in endocrine and metabolism, but those obese PCOS women were more severel. Prevention and cure for long - dated complications of PCOS were attached importance to a high degree.
