2型糖尿病患者超敏C-反应蛋白与尿微量白蛋白相关性分析

摘　　要：目的探讨2型糖尿病(T2DM)患者超敏C-反应蛋白(hs-CRP)水平与尿微量白蛋白相互关系。方法对诊断T2DM有微量白蛋白尿的91例患者(组1)、T2DM微量白蛋白尿正常的140例患者(组2)、尿微量白蛋白正常的非糖尿病137例对照者(组3)的体重指数、血压等临床资料进行调查。同时测定空腹血浆血糖(FBG)及2h血糖(PBG);空腹血清hs-CRP、胰岛素(FINS)、肌酐(Scr)、胆固醇(TCHO)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、糖化血红蛋白(HbA1C)、及尿微量白蛋白(mALB)与尿肌酐(Ucr)的比值(ACR)。结果组11ghs-CRP明显高于组2、组3(P<0.001),组2、组3间无显著性差异(P>0.05)。经Pearson相关分析显示,1ggs-CRP与年龄、TCHO、lgTG、HDL-C、LDL-C、HbA1C、lgFINS、lgACR、BMI显著正相关(P<0.05)。多元线性回归显示lghs-CRP、FBG、HbA1C、lgFINS、BMI、收缩压是尿lgACR独立影响因素(R=0.544,R2=0.296)。结论血清hs-CRP水平对T2DM患者糖尿病肾病的发生和病情监测有重要的临床价值,临床应及时监测hs-CRP水平。Objective To explore the relationship between high sensitivity C-reactive protein （hs-CRP） level and microalbuminuria in patients with type 2 diabetes. Methods 231 type 2 diabetes patients including 91 with microalbuminuria （groupl）, 140 with normal microalhuminuria （group 2）, and 137 witt. normal glucose tolerance served as normal controls （group3） were studied. Fasting blood samples are collected for hs-CRP, lipid profile, serum creatinine （Scr）, blood glucose （FBG）, insulin （FINS） and Hemoglobin A1c, （HbA1c）. Urine samples were examined for microalbumin/creatinine ratio （ACR）. Basic material are also investigated. Results The level of 1g hs-CRP in group1 are significantly higher than that in group 2 and group 3 （P〈 0.001）, and no significant differences between group 2 and group 3. The 1g hs-CRP is positively correlated with age, BMI, TCHO, 1g TG, HDL-C, LDL-C, HbA1c, 1g FINS and lg ACR. After muhivariate linear regression analysis, the 1g hs CRP,FBG, HbA1c,1g FINS,BMI were independent predictors of 1g ACR （R=0.544,R^2=0.296）. Conclusion It is important for developing and monitoring of diabetic nephropathy to examine hs-CRP level in type 2 diabetes patients.
