急性钡中毒42例心电图分析

摘　　要：目的探讨急性钡中毒时心电图异常的临床意义。方法分析42例急性钡中毒患者的心电图,其中男性19例,女性23例,年龄19个月～78岁。结果3例发生室上性心律失常,14例发生多源性和/或频发性室性早搏,2例发生心室停搏,3例发生房室阻滞和室内阻滞,由于低钾血症造成的心室复极异常26例。严重心律失常和呼吸肌麻痹是引起患者死亡的两个主要原因。结论急性钡中毒引起的心律失常是由于钡剂直接刺激心肌、中毒性心肌炎和继发性低血钾所致。Objective To assess the EKG occurring in acute barium poisoning. Methods The EGGs of 42 patients （19males, 23 females,agesranging 19/12-78yrs） suffering from acute barium poisoning were analysed. Results Supraventricular thachyarrhythmias occurred in 3 patients,frequent ventricular premature beats in 14 patients,ventricular standstill in 2 patients,atrio-ventricular block and intraventricular block in 3 patients. Ventricular repolarization abnormalities due to hypokalemia occurred in 26 patients. Severe cardiac arrhythmias and respiratory muscle paralysis werethe main causes of death in this series. Conclusion The EKG changes in acute barium poisoning are due to direct effect of barium on myocardium, toxic myocaritis and secondary hypokalemia.
