血液透析患者中心静脉插管血栓形成中尿激酶的应用与护理

摘　　要：目的观察尿激酶对血液透析患者中心静脉插管血栓进行溶栓的疗效,并总结有关的护理经验,以探讨简单、安全、有效的溶栓方法。方法收集中心静脉插管血栓形成的患者20例,在中心静脉插管血栓形成端注射尿激酶进行局部溶栓,溶栓过程分3次进行,30min后回抽血液,观察溶栓效果,并比较溶栓前后的APTT、PT、凝血酶时间、纤维蛋白原。结果溶栓成功16例,失败4例,成功率达80%,未见不良反应。溶栓前后的APTT、PT、凝血酶时间比较纤维蛋白原、无明显差异。结论采用尿激酶对血液透析患者临时性中心静脉插管堵塞者进行溶栓,操作简单,过程安全,成功率较高。能较理想地为血液透析患者重新建立一条有效的血管通路,使透析顺利进行。Objective To observe the effects of using urokinase for patients of hemodialysis by central vein, and then summarized the relative nursing points. Methods Selected 20 patients of hemodialysis by central vein, urokinase was injected in three time points, and then observed the condition of PT, APTT, TT and plasma fibrinogen. Results 16 cases were successful, the successful rate was 80%, and there were no severe, complications occurred. There were no significant differences about the PT, APTT, TT and plasma fihrinogen before and after using the urokinase. Conclusion Using the urokinase can effective improve the successful rate for patients of hemodialysis without severe complications, and then improved the quality of life for patients.
