血清细胞因子测定在慢性前列腺炎诊断、分型中的临床意义

摘　　要：目的探讨血清中细胞因子在慢性前列腺炎(CP)诊断、分型中的意义。方法用ELISA法检测146例临床诊断的慢性前列腺炎患者[其中慢性细菌性前列腺炎(CBP)组52例、慢性非细菌性前列腺炎(CAP)Ⅰ组66例、Ⅱ组28例]血清中的细胞因子,如IL-2、IL-4、IL-6、IL-8、TNF-α,同时,以20例健康男性作为正常对照,并对所取得的数据进行统计学分析。结果(1)IL-2、IL-4在CBP组、CAPⅠ组患者血清中均有不同程度的升高,与正常对照组比较差异显著(P<0．01或P<0．05),而CBP组与CAPⅠ组、CAPⅡ组与正常对照组间比较均无显著性差异(P>0．05);(2)IL-6、IL-8在CBP组、CAPⅠ组、CAPⅡ组、正常对照组间相互比较均有显著性差异(P<0．01或P<0．05);TNF-α在CBP组、CAPⅠ组、正常对照组间相互比较差异显著(P<0．01或P<0．05),CAPⅡ组与正常对照组间比较差异无显著性(P>0．05)。结论血清中IL-2、IL-4、IL-6、IL-8、TNF-α水平可能在CP的发生发展过程中起作用,对CP辅助性诊断有较大应用价值,其中IL-6、IL-8、INF-α对CP的诊断、分型有一定临床意义。Objective To investigate the significance of the determinating cytokines in serum for evaluation of chronic prostatitis（CP）. Methods The levels of interleukin-2（IL-2）, interleukin-4（IL-4）, interleukin-6（IL-6）, interleukin- 8（IL-8）, tumor necrosis factor- α （TNF- α ） in serum were detected by ELISA in 146 patients with CP[CBP52cases, CAP Ⅰ 66cases and CAP Ⅱ 28cases], and 20 healthy males as control, anti statistical analysis was performed. Results （1）The serum levels of IL-2,IL-4 in patients with CBP and CAP Ⅰ were significantly higher than those of control and difference was obvious between the patients with CBP or CAP Ⅰ and control （P〈0.01 or P〈0.05 ）, there was no significant difference between CBP, CAP Ⅰ, CAP Ⅱ and control （P〉0.05）. （2）The levels of IL-6, IL-8 were significantly different among CBP, CAP Ⅰ, CAP Ⅱ and control （P〈0.01 or P〈0.05）; the level of TNF- α were significantly different among CBP, CAP Ⅰ and control （P〈0.01 or P〈0.05）, there was no significant difference between CAP 11 and control （P〉0.05）. Conclusion The levels of IL-2, IL-4, IL-6, IL-8,TNF- α in serum play a＇role in process of the occurrence and development of CP and there are applied values to help diagnosis of CP. As to diagnosis and typing of CP, IL-6,IL-8,TNF- α are especially meaningful.
