摘 要:目的探讨坐骨海绵体肌在勃起机制中的地位和作用。为临床诊断和治疗骨盆骨折并尿道损伤后的勃起功能障碍是否与坐骨海绵体肌损伤有关提供依据。方法选用生殖期雄性大白鼠35只,随机分为手术组(25只)、假手术组(5只)和非手术组(5只)。将手术组切断双侧坐骨海绵体肌,假手术组仪显露坐骨海绵体肌后关闭切口。手术1周后选用育龄期雌性大白鼠35只,随机与上述三组——配对饲养。观察2月,了解三组配对雌鼠受孕情况。随后将手术组分为修复组(17对)和未修复组(5对),将修复组雄鼠修复其切断的双侧坐骨海绵体肌,未修复组作对照。观察2月,了解配对雌鼠受孕情况。结果手术组雄鼠死亡2只,雌鼠1只受孕,受孕率为4.35%,假手术组雄鼠死亡1只,雌鼠4只受孕,受孕率为100%,非手术组雌鼠5只均受孕,受孕率为100%,修复组雄鼠死亡9只,存活8只,其配对雌鼠2只受孕,受孕率为25%,未修复组5只雌鼠均未受孕,受孕率为0。手术组与假手术组、非手术组雌鼠受孕情况比较有统计学意义(P<0.001),假手术组与非手术组比较无统计学意义,修复组与未修复组比较无统计学意义(P>0.05)。结论切断雄鼠双侧坐骨海绵体肌必然导致勃起功能障碍,引起不育。坐骨海绵体肌在阴茎勃起机制中是勃起过程中的一个环节,不可缺少,其作用是起控制阴茎勃起血流的闸门或开关的作用。Objective To investigate the role of ischiocavernosus in erection. Methods 35 male rats were divided into 3 groups: operation group (n=25) both sides of ischiocavernosus were severed, sham operation group (n=5) two sides of ischiocavernosus were exposed and incision were sutured, non-operation group (n=5) without operation. 35 female rats at procreative period were picked out to match with them one-to-one randomly and the record of pregnancy of these mathching female rats were observed after two months. The operation group divided secondly was into repair group (n=17) and non-repair group (n=5). The severed chiocavernosus of repair group was repaired non-repair group as control group the rate of pregnancy between the two group was followed 2 month. Results Two male rats died in operation group and 1 female rats got pregnancy, the rate is 4.35%. In sham. operation group one male died and 4 female got pregnancy, the rate is 100%. In the non-operation group all of 5 female got pregnancy, the rate also 100%. In repair group 9 males died and 2 matching females got pregnancy, pregnancy rate was 25%. While the rate was 0% in non repair group. There was statistic discrepancy between operation group and non-operation group (P〈0. 001). While there was no statistic discrepancy between the operation group and non-operation group, and also between repair group and non-repair group(P〉0.005). Conclusion Severing of two sides of ischiocavernosus will inevitably lead to erectile dysfunction and infertility, ischiocavernosus are indispensable in erection, it play an important role in controlling the blood flow for penis like a sluice gate.
