摘 要:目的:探讨单纯疱疹病毒2型感染与原发性高血压的关系方法 在有1244名住院病人(年龄在35至64岁)参与的对照研究中,488例原发性高血压病人,756例对照,用ELISA方法检测血中特异性单纯疱疹病毒2型IgG抗体,进行两组问比较。本研究分析高血压的危险因素有:年龄,男性,吸烟,糖尿病,血脂紊乱,较少体力劳动,体重指数和单纯疱疹病毒2型IgG抗体阳性:在多因素分析中,应用Logistic回归分析以去除其它危险因素的影响,并计算调整后的比值比(OR值)和95%的可信区间.以表示单纯疱疹病毒2型感染和高血压的关系,及对高血压的危险性。结果 单纯疱疹病毒2型IgG抗体阳性率,高血压组病人明显多于对照组(38.3% vs.29.8%。P=0.002)。Logistic回归分忻去除其它危险因素的干预,发现单纯疱疹病毒2型IgG抗体阳性与高血压强相关,调整后的OR值为1.413(95.0%CI,1.108 to 1.802;P=0.005),调整变量包括血脂异常、糖尿病、冠心病、男性和吸烟。在单纯疱疹病毒2型IgG抗体阳性病人中,血脂紊乱的病人显著增多(P<0.05)。结论 单纯疱疹病毒2型感染可能是原发性高血压的独立危险因素。Objective: To investigate the correlation between Herpes simplex virus type 2 (HSV-2)infection and essential hypertension. Methods: In this cross-sectional study, of 1244 inpatients to Beijing Fu-Wai Hospital, Cardiovascular Institute, Chinese Academe of Medical Sciences, 488 (39.2 % )patients with essential hypertension, and 756 controls, were investigated serologically for IgG antibody to HSV-2 with ELISA. The risk factors analyzed for hypertension included age, male sex, smoking, diabetes, dyslipidemia, physical inactivity, body mass index and HSV-2 IgG seropositivity. In the multivariate analysis, the logistic regression was used to control the potential confounding factors and to calculate the adjusted odds ratios (OR)with its associated 95 % confidence intervals (CI)expressing the association of previous HSV-2 infection with the presence or absence of hypertension, and the risk of HSV-2 infection for essential hypertension. Results: The prevalence of HSV-2 IgG seropositivity was significantly higher in hypertension group than that in control group (38.3% vs.29.8%, P = 0.002) .After adjustment for confounding factors, a strong association of HSV- 2 IgG seropositivity with hypertension was found on binary logistic regression analysis, the adjusted OR of HSV-2 IgG ( + )for hypertension was 1.413 (95.0% CI, 1. 108 to 1.802; P = 0.005), adjusted eovariates included dyslipidemia, diabetes, coronary artery disease, male sex and smoking. And there were significantly more patients with dyslipidemia in HSV-2 IgG seropositive group than that in HSV-2 IgG seronegative group ( P 〈 0.05). Conclusions HSV-2 infection could be an independent risk factor for essential hypertension.
