山东省克山病病情及发病相关因素的动态观察  

Serial observation of condition and related factors of Keshan disease in Shandong

摘　　要：选择克山病重病区沂水、莒县及平邑３个县６个自然村３～１４岁人群２０９５例，自１９９０～１９９４年进行连续５年监测，同时观察３个县的急型、亚急型及慢型克山病的发病情况。结果显示监测人群１９９０、１９９２、１９９４年克山病检出率分别为２．８８％、２．４２％、２．５６％，呈下降趋势。异常心电图检出率３年依次为３．８０％、２．６０％、３．８０％。３个县无急型、亚急型发病，慢型发病６例。监测点小麦硒食量升高．土壤、玉米、瓜干硒含量无变化，人群血清及发硒食量升高。居民经济收人提高４９．１％．口粮增长１４．８％。病区居民生活改善，内环境洒水平升高，克山病发病是基本控制后的稳定势态。In order to observe the serial change of condition of Keshan disease and selenium(Se)levels in internal and external environment, 2095 cases 3￣14 year-old were selected as surveillance subjects from 6villages in Keshan disease area of Yishui,Juxian and Pingyi counties. Surveillance contents included physical checkup, electrocardiogram (ECG) and X-ray. Se content was measured in blood,hair,drinking water .soil and grain. Economic condition and brain were investigated. At same time, incidence of acute, subacute and chronic Keshan disease were surveyed for 3 counties. Results showed that the incidence of Keshan disease in 1990, 1992 and 1994 were 2. 88%, 2. 42% and 2. 56%, respectively, on surveillance subjects and that the incidence decreased. Incidence of abnormal ECG on surveillance subjects, in 1990, 1992 and 1994 were 3. 80%, 2. 60% and 3. 80%, respectively. Acute and subacute Keshan disease were not found, but 6 patients with chronic Keshan disease occurred in 3 counties. Se content of drinking water decreased,Se content of wheat was inereased,but Se contents of soil,maize and sweet potato did not change in external environment. Se contents of blood and hair increased in internal environment. Economic condition and main grain of inhabitant were improved significantly. Condition of Keshan disease was stable in Shandong.The life quality of inhabitant in endemic area was improved.Levels of Se in blood and hair risen. Therefore epidemic of Keshan disease was not nearly possible.
