西安地区部分健康体检人群血脂水平分析

摘　　要：目的完善西安地区心脑血管疾病高危人群血脂参考值的建立,促进临床对血脂指标的更合理应用。方法对西安地区1 011例健康体检人群血清胆固醇(CHO)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白A(A poA)、载脂蛋白B(A poB)进行分析研究,观察血脂水平的变化趋势。结果西安地区健康体检人群血脂水平呈逐年上升趋势,发生年龄呈年轻化趋势。其中TG,HDL-C,LDL-C升高最明显,与原参考值比较有极显著差异(P<0.01)。且发病年龄提前,女性23 y,男性29 y。不同地区,不同年龄,不同性别,不同职业之间均有差异(P<0.05或P<0.01)。中老年人群特别是50 y以上年龄组,女性TG水平明显高于男性(P<0.01),70 y以上年龄组无论男女均有所下降。人群血脂异常检出率LDL-C>TG>A poB>A poA>CHO>HCL-C,增高的顺序由大到小依次为机关干部>教师>工人>医生。结论西安地区健康体检人群血脂水平明显升高,较原参考值发生了改变,应根据不同年龄与性别建立各自的血脂参考值范围,有效地避免漏诊和误诊,为心脑血管疾病筛查和防治提供科学的依据。Objective The study was to analyze the blood fat reference value within high-risk group of cardiovascular and cerebrovascular diseases in Xi＇an area. Methods To investigate the tendency of the level of blood fat indexes including CHO, TG, HDL-C, LDL-C,Apo-A,Apo-B of 1 011 cases selected randomly from the healthy crowd through the normal physical examination in Xi＇an area,which were detected by the high-level automatic biochemical analyzer （7600-020）. Resuits The blood fat level of the healthy crowd in Xi＇an area has been increasing year by year and the age of onset has been becoming earlier. Especially among all the blood fat indexes ,the levels of TG,HDL-C and LDL-C have been increasing ob- vlously（P〈0. 01） ,which had a significent difference from the original reference values（P〈0. 01）. There is also a disparity between different area,ages,genders and profession（P〈0. 05 or P〈0. 01）. The TG level in adult female is higher than in adult males within the people of middle and old age,especially the over-50 age group,but the level decreases within the over-70 age group whatever adult males or adult female. As to the abnormality recall ratio,LDL-C is the highest in all the indexes ,then TG ,Apo-B,Apo-A ,CHO,HDL-C by turns ,especially in the crowd of the officials ,teachers ,workers ,doctors by turns. Conclusions The blood fat level in healthy crowd is higher than the original reference value in Xi＇an area. It＇s necessary to establish new blood fat reference range according to age ,profession ,and gender.
