摘  要:目的:了解成都地区健康人群腰椎侧位、侧位中部区域骨密度变化规律,建立该型仪器在成都地区骨密度正常参考值。方法:检查于2003-12/2004-12在四川省人民医院核医学科完成。①采用美国LUNAR公司的EXPERT-XL双能X射线骨密度仪测定成都地区健康体检者703名,其中男269名,女434名,测量部位包括腰椎前后位L1~4和腰椎侧位L2~4骨密度。②图形分析上,感兴趣区划定采用腰椎侧位全椎体和侧位中间区域两种方法,中间区域大小约为全椎体长宽的1/2。③按年龄、性别分别输入数据,以10岁为1个年龄组,包括20~29岁,30~39岁,40~49岁,50~59岁,60~69岁,70~79岁,80~等7个年龄段,分别计算各组骨密度值和骨密度累积丢失率。结果:703名均完成测试进入结果分析。①男性骨密度峰值出现在30~39岁[腰椎侧位:L2~4(0.917±0.118),(0.879±0.160),(0.967±0.162)g/cm2;侧位中部:L2~4(0.910±0.112),(0.786±0.157),(0.897±0.155)g/cm2],女性骨密度峰值出现在20~29岁[腰椎侧位:L2~4(0.830±0.144),(0.781±0.169),(0.808±0.171)g/cm2;侧位中部:(0.783±0.125),(0.704±0.201),(0.742±0.172)g/cm2],无论男女,随着年龄增加,骨密度逐渐降低。②男性及女性腰椎侧位及侧位中部骨密度累积丢失率明显高于腰椎前后位;女性腰椎骨密度累积丢失率明显高于男性,尤其是50岁之后女性腰椎侧位及侧位中部骨密度累积丢失率达18.8%~50.8%,男性同期为8.6%~40.3%。结论:①得出了EXPERT-XL双能X射线骨密度仪测定的成都地区人群骨密度的正常参考数据。②女性在50岁后更应注意预防骨质疏松。③腰椎侧位及侧位中部骨密度较前后位检测具有更高的敏感性。AIM: To analyze the change law of bone mineral density (BMD) of lateral and midlateral lumbar spine of healthy people and build normal data of BMD in Chengdu area, METHODS: The investigation was performed at the Department of Nuclear Medicine, Sichuan Provincial People's Hospital from December 2003 to December 2004. ①BMD of anterior and posterior lumbar spine (L1-4) and lateral lumbar (L2-4) were measured with LUNAR EXPERT-XL dual energy X-ray absorptiometry in 703 (269 men and 434 women ) normal subjects. ②For analysis of the lateral spine images, the ROI in midlateral lumbar was set as an area of half the length and the width of the vertebral body. ③Data of BMD were analyzed according age and sex by 10 years a group of male and female respectively, including 20-29, 30-39, 40-49,50-59,60-69, 70-79, 80-year groups, BMD and the accumulative bone loss percentages of each group were calculated, respectively. RESULTS: Totally 703 people were examined and involved in the result analysis. ①The peak bone mass (PBM) of male was in 30-39 years group, and BMD of lateral and midlateral L2-4 were (0.917±0.118), (0.879±0.160), (0.967±0.162) g/cm^2 and (0.910±0.112),(0.786±0.157), (0.897±0.155) g/cm^2, respectively. The PBM of female was in 20-29 years group, and BMD of lateral and midlateral L2-4 were (0.830±0.144), (0.781±0.169), (0.808±0.171) g/cm^2 and (0.783±0.125), (0.704±0.201), (0.742±0.172) g/cm^2, respectively. BMD declined with increasing age, no matter male or female. ②The accumulative bone loss percentages of lateral and midlateral lumbar spine were much higher than those of pesterior-anterior lumbar spine and in female were much higher than those in male. The accumulative bone loss percentages of lateral and midlateral lumbar spine after 50 years of age were 18.8%--50.8% in female and 8.6%--40.3% in male.
