摘 要:目的:分析活性氧与骨基质的相关性,以进一步阐明活性氧与骨质疏松症的内在联系。方法:选择深圳市中医院骨科2003-01/12的女性住院患者90例,所有患者年龄<70岁,绝经1年以上。应用双能X射线骨密度测量仪(QDR-4500A型、美国HOLOGIC公司生产)测定其骨密度,将骨密度低于0.790g/cm2者归于骨质疏松组,骨密度0.790~0.897g/cm2者归于骨量减少组,骨密度不低于0.897g/cm2者归于正常组,每组30例。检测各组血清丙二醛、超氧化物歧化酶及I型胶原羧基端肽水平,用SPSS10.0软件进行直线回归及相关分析,分析各指标间的相关性。结果:90例全部进入结果分析。①丙二醛浓度:骨质疏松组和骨量减少组高于正常组[(11.98±2.12),(9.28±1.44),(7.60±1.70)μmol/L,P<0.01],骨质疏松组高于骨量减少组(P<0.01)。②超氧化物歧化酶活性:骨质疏松组和骨量减少组低于正常组[(722.61±105.10),(882.34±102.38),(1063.06±76.68)μkat/L,P<0.01],骨质疏松组低于骨量减少组(P<0.01)。③I型胶原羧基端肽水平:骨质疏松组和骨量减少组高于正常组[(10.90±1.96),(7.89±1.78),(5.47±1.58)μg/L,P<0.01],骨质疏松组高于骨量减少组(P<0.01)。④丙二醛与I型胶原羧基端肽呈显著正相关(r=0.809,P<0.01),而超氧化物歧化酶与I型胶原羧基端肽呈显著负相关(r=-0.883,P<0.01)。结论:绝经后骨质疏松患者体内的活性氧水平较正常人增高,骨基质的降解也增多,提示活性氧能促进骨基质的降解,加速骨吸收,可能参与和促进了骨质疏松症的病理过程。AIM: To analyze the correlation between reactive oxygen species and bone matrix, so as to further clarity their internal relation. METHODS: A total of 90 female inpatients aged 〈 70 years with more than one year of postmenopause in Department of Orthopedics, Shenzhen Traditional Chinese Medicine Hospital from January to December 2003 were selected. The bone mineral density (BMD) was examined with the Dual Energy X-ray Absorptiometry (QDR-4500A, HOLOGIC Incorporation of U,S), and those with BMD 〈 0.790 g/cm^2 were enrolled in osteoporosis group, those with BMD between 0.790-0.897 g/cm^2 in bone mass reduction group and those with BMD 〉 0.897 g/cm^2 in normal group with 30 cases in each group. Then their serum malondialdehyde (MDA), superoxide dismutase (SOD) and type I collagen telopeptide (ICTP) were detected; the SPSS 10.0 software was used for linear regression and related analysis, and the correlation of MDA and SOD with ICTP was analyzed. RESULTS: All the 90 case were involved in the result analysis. ① Serum MDA level: The osteoporosis group and bone mass reduction group were higher than the normal group [(11.98±2.12), (9.2823±1.44), (7.60±1.70) lumol/L, P〈0.01], and the osteoporosis group was higher than the bone mass reduction group (P〈0.01).② Serum SOD level: The osteoporosis group and bone mass reduction group were lower than the normal group [(722.61±105.10), (882.34±102.38), (1063.06±76.68) μkat/L, P 〈 0.01], and the osteoporosis group was lower than the bone mass reduction group (P 〈 0.01). ③ICTP level: The osteoporosis group and bone mass reduction group were higher than the normal group [(10.90±1.96), (7.89±1.78), (5.47±1.58)μg/L, P 〈 0.01], and the osteoporosis group was higher than the bone mass reduction group (P 〈 0.01). ④MDA level was positively correlated with ICTP (r=0.809, P 〈 0.01), but the SOD level is negatively correlated with ICTP (r=-0.883, P 〈 0.01).
