冠状动脉粥样硬化性心脏病心绞痛患者血浆标记物特征

摘  要:目的:总结近10年来国内外学者对冠状动脉粥样硬化性心脏病(冠心病)心绞痛血浆标记物的研究成果,探讨血浆标记物在冠心病心绞痛发生、发展过程中的变化规律,以及在心绞痛诊断、危险度分层及预后判断中的意义,为临床早期诊断和早期干预提供依据。资料来源:应用计算机检索Medline1990-01/2005-12关于冠心病血浆标记物的文章。检索词“Anginapectoris,biologymark”,并限定文章的语种类为English。同时利用计算机检索清华全文期刊数据库1995-01/2005-12的相关文章,限定文章语言种类为中文,检索词“心绞痛,生物学标记”。资料选择:对资料进行初审查,选择血栓素B2与6酮前列腺素F1α、纤维蛋白原、组织型纤溶酶原激活物和纤溶酶原激活剂抑制物、D-二聚体、CD62P与CD63、内皮素与降钙素相关肽、C反应蛋白、白细胞介素6和白细胞介素8等血浆标记物相关的文献。排除综述类文献和重复性研究文献。资料提炼:共收集到相关文献141篇,34篇符合上述标准。其中血栓素B2与6酮前列腺素F1α相关文献1篇,纤维蛋白原相关文献3篇,组织型纤溶酶原激活物和纤溶酶原激活剂抑制物相关文献5篇,D-二聚体相关文献5篇,CD62P与CD63相关文献3篇,内皮素与降钙素相关肽相关文献3篇,C反应蛋白相关文献6篇,白细胞介素6和白细胞介素8相关文献8篇。资料综合:研究表明,心绞痛发作期间,血栓素B2及血栓素B2/6酮前列腺素F1α比值显著升高,降钙素相关肽浓度则显著下降;不稳定性心绞痛患者血D-二聚体、CD62P,CD63、内皮素、组织型纤溶酶原激活物和纤溶酶原激活剂抑制物、白细胞介素6、白细胞介素8等含量显著升高,且与病情严重程度明显相关;不稳定性心绞痛患者的血纤维蛋白原、D-二聚体、组织型纤溶酶原激活物、白细胞介素6、白细胞介素8、C反应蛋白水平持续升高,常提示今后发生心脏事件�OBJECTIVE: To summarize new achievements on plasma markers performed by domestic and foreign scholars in patient with angina pectoris of coronary heart disease in recent 10 years, and probe into the changing rules of onset and developing process of angina pectoris of coronary heart disease as well as the significance in angina pectoris diagnosis, risk stratification and prognostic judgment, so as to provide basis for early clinical diagnosis and early intervention. DATA SOURCES: English articles related to plasma marker of angina pectoris published between January 1990 and December 2005 were searched in Medline by computer with the key words of "angina pectoris, biology mark". Meanwhile, relevant Chinese articles between January 1995 and December 2005 were searched in Tsinghua Full-text Periodical Database by imputing the key words of "angina pectoris, biology mark". STUDY SELECTION: Data were checked in the first trial, and literatures related to thromboxane B2 (TXB2) and 6-Keto-PGF1α, fibrinogen, tissue type activator of plasminogen, plasminogen activator inhibitor, D-dimer, CD62P and CD63, endothelin and calcitonin generelated peptide (CGRP), C-reactive protein, interleukin-6 and interleukin-8 etc. Reviews and repetitive studies were excluded. DATA EXTRACTION: A total of 141 relevant articles were collected, out of which 34 articles were in accordance with above-mentioned criteria. In 34 articles, 1 article was related to TXB2 and 6-Keto-PGF1α, 3 articles were related to fibrinogen, 5 articles were related to tissue type activator of plasminogen and plasminogen activator inhibitor, 5 articles were related to D-dimer, 3 articles were related to CD62P and CD63, 3 articles were related to endothelin and CGRP, 6 articles were related to c-reactive protein and 8 articles were related to interleukin-6 and interleukin-8. DATA SYNTHESIS: Studies showed that the plasma density of TXB2, TXB2/6-Keto-PGF1α were elevated significantly, while the concentration of CGRP decreased remarkably when a
