阻塞性睡眠呼吸暂停综合征患者血浆超敏C反应蛋白和IL-6水平的观察

摘　　要：目的观察阻塞性睡眠呼吸暂停综合征(OSAS)患者血浆超敏C反应蛋白(hsCRP)与白细胞介素6(IL-6)水平,及其对于无创正压通气(nCPAP)的反应。方法选择多导联睡眠监护(PSG)证实为OSAS的25例患者作为实验组,另外15例无OSAS的健康者为对照组,观察两组清晨血浆hsCRP与IL-6差异,观察实验组血浆hsCRP、IL-6与睡眠呼吸暂停指数(AI)的关系、nC-PAP治疗1个月后的变化。结果OSAS患者血浆hsCRP、IL-6明显高于对照组(P均<0.05),hsCRP、IL-6与睡眠AI均呈现出较好的相关性(P均<0.05),但与IL-6不同,OSAS患者清晨与夜间睡前CRP浓度比较差异无显著性(P>0.05)。经nCPAP治疗后OSAS患者血浆hsCRP与IL-6浓度均显示显著降低(P均<0.05)。结论OSAS患者血浆hsCRP、IL-6水平明显升高,且可通过nCPAP治疗得到降低,相比IL-6、hsCRP更能反映OSAS患者慢性炎症状态。Objective To observe serum levels of hsCRP and IL-6 in patients with obstructive sleep apnea syndrome（OSAS） and the effect of nasal continuous positive airway pressure（nCPAP） on them. Methods 25 patients with OSAS proved by polysomnography （PSG） and 15 cases without OSAS were selected as observation and control group respectively. Morning and night （before sleeping） serum levels of hsCRP and IL-6 were compared in OSAS group. In addition, the effects of 1 month of nCPAP treatment on hsCRP and IL-6 were studied in patients with OSAS. Results Levels of hsCRP and IL-6 were significantly higher in patients with OSAS than that in control subjects （P 〈 0.05 ）, and there was a positive correlation between level of hsCRP and IL-6 and apnea index（AI） （P 〈 0.05 ）. Unlike IL-6, there was no significant difference between morning and night level as far as hsCRP was concerned （ P 〉 0.05 ）. After one month of nCPAP treatment beth hsCRP and IL-6 were shown significantly decreased. Conclusions Levels of hsCRP and IL-6 are elevated in patients with OSAS but are decreased by nCPAP. Compared with IL-6, hsCRP is a more worthy marker reflecting chronic inflammation in patients with OSAS.
