摘 要:目的:探讨中药贞清方对2型糖尿病大鼠肾脏病变的防治作用及其机制。方法:采用高糖高脂饲料喂养加小剂量链脲佐菌素注射的方法建立2型糖尿病模型。将成模大鼠随机分成模型组,贞清方高、低剂量组,并设立正常对照组。治疗持续8周。研究贞清方对血糖、尿微量白蛋白排泄率(UAER)、肾质量指数、肾脏病理改变和图像分析结果的影响,并用免疫组织化学方法检测贞清方治疗后基质金属蛋白酶-9在肾脏的表达情况。结果:贞清方能显著降低糖尿病大鼠的空腹血糖和餐后血糖,贞清方高、低剂量组的UAER、肾重指数较模型组明显降低(P<0.01,P<0.05),而基质金属蛋白酶-9的表达明显增强(P<0.01,P<0.05)。病理检查结果:模型组HE染色显示肾小球体积增大,PAS染色显示系膜区基质中度增生以及毛细血管基底膜增厚;贞清方组上述病理变化较模型组明显减轻。图像分析显示:贞清方高、低剂量组肾小球细胞外基质相对面积较模型组降低(P<0.01,P<0.05)。结论:贞清方能明显改善2型糖尿病大鼠的肾脏病理改变、降低UAER,以高剂量组的作用更明显,可能与其降低血糖及调节基质金属蛋白酶-9的表达有关。OBJECTIVE To explore the preventive and therapeutic effects of Chinese herbal formula Zhenqing fang (ZQF) on renal lesions in experimental type 2 diabetic rat. METHODS Type 2 diabetic rat modal was induced by intraperitoneally giv ing small dose streptozotocin after feeding high sucrose and high fat diets for one month. The diabetic rats were randomly divid ed into untreated group(untreated group), the ZQF in high and low dosage treated groups (high and low-dosage groups). Normal control group(control group) was established. The course of treatment continued 8 weeks. Blood glucose, urinary microalbumin excretory rate(UAER) and kidney weight index were measured. Renal pathology and microimage results were ana lysed. Immunohistochemistry was used to detect the expression of matrix metalloproteinase-9 in renal tissue. RESULTS ZQF could remarkably lower the fasting blood glucose and postprandial blood glucose of diabetic rats. Compared with the untreated group, UAER and kidney weight index in high and low-dosage groups were obviously lowered (P〈0. 01, P〈0. 05),and the expression of matrix metalloproteinase-9 was markedly increased (P〈0. 01, P〈0. 05). In untreated group, renal tissue morphology indicated enlargement of glomeruli with HE staining, and proliferation of mesangial matrix and incrassation of capillary basement membrane by PAS staining. While in ZQF groups, those morphological changes were significantly improved. Microimage analysis showed that the level of glomerular extracellular matrix in high and low-dosage groups were less than that in untreated group (P〈0. 01, P〈0. 05). CONCLUSION ZQF is effective in improving renal lesions and decreasing UAER in diabetic rats, and the above effects of high dosage group are better than low-dosage group, those may he correlated with its role of decreasing blood glucose and modulating the expression of matrix metalloproteinase-9.
