经软骨闭合入路矫正陈旧外伤性歪鼻畸形

摘　　要：目的:针对陈旧外伤性歪鼻畸形的具体临床症状,选择合适歪鼻矫正方法,提高临床治疗效果。方法:2003年3月￣2006年8月,对无明显通气障碍、鼻窦炎的23例陈旧外伤性歪鼻畸形患者,采用鼻外侧软骨闭合入路联合鼻小柱旁联合切口,在局麻或全麻下施行了鼻骨复位、鼻中隔矫正术。结果:对上述患者随访3￣12个月,优17例,良6例,鼻畸形矫正满意,外形恢复良好。结论:对无明显通气障碍、鼻窦炎轻度陈旧外伤性歪鼻畸形患者,该术式入路短,操作简单,损伤小,术后恢复快,值得推广。Objective To investigate the accurate techniques for treating posttraumatic deviated nose deformity according to the different clinical symptoms and improve the efficacy of clinical treatment. Methods From March 2003 to August 2006, under the general or local anesthetic, the closed transcartilaginous approach combined the columella incision was employed on 23 patients without obstruent ventilation and sinuitis, which reduced the nasal bone and straightened the septum while maintaining them in the anatomic position. Results 23 patients with posttraumatic deviated nose deformity were managed with this measure. The period of follow-up ranged from 3 months to 1 year, the outcome was excellent in 17 cases and good in 6 cases. The nasal deformities were corrected satisfactorily and normal nasal shapes were obtained. Conclusion The method possesses the advantages of simplicity, the short approach, precision, minimal trauma, and especially is useful for the patient without obstruent ventilation and sinuitis.
