仁术健胃颗粒对慢性萎缩性胃炎癌前病变患者胃癌相关基因的影响

摘　　要：目的探讨仁术健胃颗粒对慢性萎缩性胃炎(CAG)癌前病变的治疗作用及机理。方法190例CAG癌前病变患者随机分为两组,治疗组(128例)给予仁术健胃颗粒口服,对照组(62例)给予胃复春片口服,疗程均为6个月。观察两组患者临床疗效、内镜、病理疗效及治疗前后p53、Bcl-2表达积分值的变化。结果治疗组临床总有效率为91.4%,胃镜总有效率70.3%,对胃黏膜萎缩、肠化、异型增生的总有效率分别为64.1%、66.4%、67.9%,并能降低其病理积分值。免疫组化结果显示,仁术健胃颗粒能显著抑制p53、Bcl-2的表达(P<0.05)。结论仁术健胃颗粒对CAG癌前病变的临床症状、胃镜征象、病理变化有显著改善作用,其机理可能与抑制癌基因的激活和抑癌基因的失活,促进胃癌前细胞凋亡有关。Objective：To explore the therapeutic effect and mechanism of Renzhu Jianwei Granule （RZJWG） in treating chronic atrophic gastritis（CAG） precancerous lesion. Methods：One hundred and ninety cases of CAG precancerous lesion were randomly divided into two groups, The treatment group （n=128） were treated with oral administration of RSJWG and the control group（n= 62） with oral administration of Weifuchun Tablet （WFCT）, for 6 months. The clinical effect ,gastroscopic and pathological effects ,and scores for p53 and Bcl-2 expression were observed before and after treatment in both groups. Results：The total effective rate was 91.4 % and the effective rate for gastroscope was 70.3%;The effective rate for gastric mucosa atrophy,intestinal metaplasia ＋dysplasia was 64.1 %, 66.4% and 67.9% ,respectively,and the pathological scores were significantly lowered after treatment in the treatment group. Immunohistochemical analysis showed that RZJWG could significantly inhibit the expression of p53 and Bcl 2 with significant differences （P,0. 05）. Conclusion： RZJWG can significantly improve the clinical symptoms, gastrocopic and pathological changes in CAG precancerous lesion. The mechanism is possibly related with inhibition of activization of oncogene and inactivization of suppressor gene to promote apoptosis of precancerous cells.
