祛瘀消斑胶囊对易损斑块的稳定作用

摘　　要：目的应用血管内超声技术观察祛瘀消斑胶囊对易损斑块的稳定作用。方法23只兔给予球囊损伤腹主动脉加高脂饲料喂养,12周末20只造模兔随机分为自然消退组和中药组,每组10只。24周末在腹主动脉斑块处转染携带人野生型p53基因,两周后给予中国斑点蝰蛇毒和组胺触发斑块破裂。于12周末和26周末分别进行血脂和血管内超声检测,并和病理及电镜检测结果对比。结果治疗后中药组实验兔血清总胆固醇和低密度脂蛋白胆固醇均显著低于自然消退组(P<0.05或P<0.01);中药组实验兔血管外弹力膜面积、斑块面积和管腔面积狭窄百分率较造模后显著降低(P<0.05或P<0.01);造模后、自然消退组和中药组实验兔形态学检查有明显差异。结论祛瘀消斑胶囊能明显增加易损斑块的稳定性,血管内超声技术是定性和定量判断斑块易损性的可靠方法。Objective, To observe the stabilizing action of Quyu Xiaoban Capsule on vulnerable plaque by using intravascular ultrasound （IVUS）. Methods：Twenty-three rabbits with balloon-induced abdominal aortic wall injury were fed with high fat diet. At the end of 12th week, 20 of them were divided randomly into a natural regression group and a Chinese medicine group, 10 for each group. At the end of 24th week ,recombinant adenovirus carrying human wild type p53 gene were injected via a catheter into the aortic segments rich in plaques. Two weeks later, the plaque rupture was triggered by Chinese Russell＇s Viper Venom and histamine. At the end of the 12th and 26th week ,blood samples were taken to determine serum lipids and the abdominal aortas were detected with IVUS. The results were compared with pathological and electron microscopic results. Results：Compared with the natural regression group,total cholesterol（TC） and low density lipoprotein-cholesterol（LDL-C） in Chinese medicine group were significantly lower （P〈 0. 05 and 0.01, respectively）. Extra-vascular elastic membrane area （EEMA）, plague area （PA） and lumen area stenosis percent （LAS%） in Chinese medicine group were significantly lower than those before treatment （P〈0.05 or P〈0.01 ）, with significant differences in morphological examination. Conclusion：Quyu Xiaoban Capsule can promote stabilization of vulnerable plaques. IVUS technique provides a reliable method for assessing the vlnerability of AS plaque.
