部分脾栓塞术结合益气健脾活血中药治疗肝硬化脾功能亢进的研究

摘　　要：目的观察部分脾栓塞术(PSE)结合益气健脾活血中药治疗肝硬化脾机能亢进的疗效。方法将68例脾功能亢进患者随机分为2组,治疗组36例采用PSE结合益气健脾活血中药治疗,对照组32例单行PSE治疗,均随访1 a。结果2组血小板计数均逐渐上升,治疗组上升幅度较对照组大,15 d达高峰,以后逐渐下降,3个月后渐稳定;白细胞近期急剧增高,15 d后逐步下降,3个月后趋向稳定;红细胞上升缓慢,治疗组上升幅度较对照组大,6个月后趋向稳定。脾动脉、脾静脉及门静脉直径2组均有不同程度缩小,近期疗效无显著性差异(P>0.05)。结论PSE结合益气健脾活血中药治疗脾功能亢进疗效好,值得临床推广应用。Objective It is to observe the efficacy of partial spleen embolization （PSE） combined with traditional Chinese medicine （TCM） of nourishing Qi and improving spleen and activating circulation on hypersplenism complicating with liver cirrhosis. Methods 68 patients with hypersplenism were randomly divided into treatment group and control group. Treatment group （36 cases） was treated by PSE combined with TCM of nourishing Qi and improving spleen and activating circulation. Patients （32 cases） in control group were treated with PSE only. All the patients were followed up for one year after treatment. Results The platelet count of the patients in the two groups began to increase after treatment, and that increased more in treatment group. The increasing range got to the extremum after 15 days, then the number of platelet decreased little by little and was stable three months later. Red blood cell began to increase slowly after treatment, and the increasing range in treatment group was higher than that in control group. The number of red blood cell effected stabilization after six month. The diameter of spleen vein, spleen artery and portal vein deflated were contracted in both groups, but there was no significant difference in the short- term efficacy compared with each other （P 〉 0.05）. Conclusion PSE combined with TCM of nourishing Qi and improving spleen and activating circulation has a good effect on hypersplenism complicating with liver cirrhosis, which is worthy to be applied widely in clinic.
