摘  要:目的观察腺苷超声心动图负荷试验(ATE)中心电图(ECG)、血流动力学参数的变化及腺苷负荷的安全性。方法59例入选患者分成3组,分2阶段静脉持续泵入腺苷6min,观察二维超声心动图(UCG)室壁运动变化,同时观察并记录心电图、血流动力学参数的变化及不良反应的发生情况。结果(1)负荷后3组患者的心电图出现了不同程度的变化,负荷ECG对冠心病的检出率为33·3%;(2)用药剂量达140μg·kg-1·min-1阶段时,Ⅰ组和Ⅲ组患者心率增加、血压下降,与静息状态比较差异均有统计学意义(P<0·05);Ⅱ组患者注射腺苷后心率与静息时比较差异有统计学意义(P<0·05),但收缩压变化与静息时比较差异无统计学意义(P>0·05);(3)负荷后Ⅲ组患者的峰值速度比值(E/A)与静息时比较差异无统计学意义(P>0·05),但射血分数(EF)值与静息时比较差异有统计学意义(P<0·01)。结论腺苷负荷心电图诊断冠心病敏感性低于ATE;腺苷负荷试验是安全的。Objective To investigate the changes of hemodynamlc parameters and electrocardiogram (ECG) of adenosine stress echocardiography (ATE), and the security of adenosine stress. Methods Totally 59 enrolled patients were divided into 3 groups and given injection of adenosine continuously for 6 minutes. The changes of wails motion in UCG and hemodynamic parameters, as well as the state of adverse reactions were observed. Results After the stress, there occurred a series changes in ECG for all patients in the three groups, and the detectable rate of coronary heart disease (CHD) by stress ECG was 33, 3%. Compared with the state still, the heart rate increased and blood pressure fell, and the differences were significant during the injection of 140 μg ·kg^-1 ·min^-1 in the groups Ⅰ and Ⅲ ( P 〈0, 05) ; the heart rate had significant difference ( P 〈0, 05), but the blood pressure had no significant difference in the group Ⅱ ( P 〉 0, 05 ), and there was a significant difference in ejection fraction in the group Ⅲ ( P 〈 0. 01 ). There was no severe adverse effects in the adenosine stress. Conclusion The sensitivity of adenosine stress ECG is lower than that of ATE, and the adenosine stress test is safe.
