摘　　要：目的探讨改良神经内窥镜辅助下手术治疗高血压性脑出血的效果。方法对16例高血压性脑出血患者于发病24h内行改良神经内窥镜下血肿清除术,即通过透明导管鞘进入内窥镜及常规吸引器进行血肿清除。结果术后3d复查脑CT示血肿全部清除10例,血肿大部清除(>90%)4例。手术效果满意。结论改良神经内窥镜辅助下手术引入透明导管鞘可极大改善内窥镜下的视野;应用常规吸引器可提高血肿的清除率及精确度。为内窥镜在高血压性脑出血微创手术中的应用提供了新的前景。[Objective] To introduce a new method of endoscope assisted surgery for hypertensive intraeerebral haematoma. [Methods] Endoscopic evacuation of hypertensive intraeerebral haematomas were performed in 16 patients. An endoscope was guided by a transparent sheath, through which suction was introduced for aspiration of haematoma. [＇Results] Complete evacuation of haematoma was achieved in ten cases, near-total evacuation in four eases. Patients recovery were excellent. [Conclusion] The transparent sheath provided clear vision under endoscope and facilitated accurate and efficient evaeulation of the hematoma.
