摘　　要：目的探讨在三维痫灶精确定位诊疗计划系统定位下,联合应用多种术式治疗致痫灶较广泛的原发性顽固性癫痫的方法。方法对138例经影像学检查及三维痫灶精确定位诊疗计划系统精确定位后,联合应用多种术式治疗的致痫灶较广泛的原发性顽固性癫痫患者的临床资料、手术方案、手术效果进行分析总结。结果所有病例随访1年以上,随访疗效评估效果达到良好以上的占71.7%,总有效率达到92.0%,术后神经功能良好,无明显后遗症。结论经三维痫灶精确定位诊疗计划系统精确定位痫灶后,恰当选用多种术式联合治疗致痫灶较广泛的原发性顽固性癫痫疗效较好。Objective To study the curative effect of combined operations for idiopathic and intractable epilepsy with wide epileptic focus with the help of three-dimensional epileptogenic toci localizauon and treatment planning system. Methods After the localization of epileptic loci by imaging examination and three-dimensional epileptogenic loci localization and treatment planning system, 138 idiopathic and intractable epileptic patients were treated with the combination of various operative methods, and their clinical data, operative methods and therapeutic outcomes were summed up and analyzed. Results The follow-up of over 1 year postoperatively revealed 71.7% of the cases obtained favorable therapeutic effects and the total efficacy rate was 92.0%, with satisfactory nerve function preservation and no obvious sequela. Conclusion With the help of three-dimensional epileptogenic loci localization and treatment planning system, the proper combined operations are safe and effective for idiopathic and intractable epilepsy with wide epileptogenic focus.
