摘 要:目的应用冠脉血流显像(CFDI)技术评价缺血预适应(IP)对犬心肌缺血再灌注(IR)后冠脉血流储备(CFR)的影响。方法将14条杂种犬随机分为两组:IR组,持续缺血3h后再灌注2h;IP+IR组,在持续缺血前进行4次短暂性缺血5min,中间间隔5min再灌注。采用左前降支动脉(LAD)套扎建立IP开胸犬模型,分别于基础状态和再灌注2h测量两组CFR;处死犬后评价心肌纤维酶活性与超微结构的改变。结果两组犬基础状态时CFR值无显著性差异,再灌注2h的CFR值均较基础状态时减低;再灌注2h时IR组CFR值低于IP+IR组。心肌纤维酶活性与透射电镜均显示IP+IR组较IR组心肌损伤减轻。结论应用CFDI可以发现IP能够改善IR后CFR,减轻心肌损伤。Objective This study was performed to evaluate the effects of ischemic preconditioning (IP) on coronary flow reserve (CFR) after ischemia and reperfusion (IR) by coronary flow Doppler imaging. Methods Fourteen mongrel dogs were divided into two groups: IR group and IP+IR group. LAD was occluded for 3 hours, then the heart was reperfused for 2 hours. CFDI was performed at baseline and after 2 hours reperfusion in both groups and CFR was also measured. After the experiment, the myocardium was dyed respectively with Evan's blue and triphenyl tetrazolium chloride (TTC) solution. The risk area and necrotized zone were determined. Tissue samples were taken from the risk area into the fix solution for the electronic microscope examination. Results CFRs determined by CFDI after 2 hours reperfusion were significantly lower than those at baseline in the two groups. And CFR in the IP+IR group was significantly higher than that in the IR group. Under the electronic microscopy the myocardium in the risk area in IR group was destructed seriously. Conclusions CFDI may prove the protective effects of IP on CFR and myoeardium damage after IR.
