慢性充血性心力衰竭患者血清TNF-α、IL-6RIA的临床意义

摘　　要：目的研究慢性充血性心力衰竭(chroniccongestiveheartfailure,CHF)患者血清中肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)放射免疫分析(radioimmunoassay,RIA)的价值。方法用放射免疫分析测定176例CHF患者和30例健康人血清TNF-α、IL-6水平。比较分析CHF患者与正常人以及不同心功能程度的CHF患者之间TNF-α、IL-6水平的变化。结果CHF患者血清TNF-α、IL-6水平明显高于正常对照组(P<0.05),且均随着心衰程度的加重而增高,呈高度正相关。不同程度心衰患者之间TNF-α、IL-6水平均有明显差异(P<0.01)。结论CHF患者存在免疫激活和心肌炎症反应,心衰程度越重,炎症反应也越明显。放射免疫分析测定TNF-α、IL-6对CHF病程及治疗疗效、预后判断具有重要的价值。Objective To study the clinical significance of the detection of changes of serum TNF - α and IL - 6 levels in patients with chronic congestive heart failure. Methods The serum levels of TNF- α and IL- 6 were determined with RIA in 176 patients with chronic heart failure （CHF） and 30 controls. Results The serum levels of TNF - α and IL - 6 in patients with CHF were significantly higher than those in controls （P 〈 0. 05 ）. The levels increased along with the increase of severity of cardiac failure. The levels in patients with cardiac function of any grade were significantly different from those in patients with another grade of cardiac fun ction. Conclusion Immunological activation and myocardial inflammation exist in CHF patients. The increasing levels of TNF - α and IL -6 can trigger the onset and development of CHF.
