摘　　要：目的股骨远端骨折均可以造成骨和周围软组织复杂性损伤，保守治疗常常造成对线不良，骨折不愈合及膝关节的僵直，而且切口大，出血多，损伤重，患者恢复时间长。为了减少这些问题，我院开展了微创内固定系统（LISS）治疗。方法对5例股骨远端骨折患者进行手术配合，包括术前探访、置仰卧位于骨科手术牵引床进行双下肢牵引复位、C-臂X线机透视检查复位情况、手术中提供LISS专用器械作骨折内固定等。结果5例手术配合顺利，切口I期愈合，X线片复查骨折对位、对线良好。随访时间6～10个月，平均8个月。膝关节功能按MerChan等评分标准，优3例，良2例，优良率为10％。结论LISS是一种治疗股骨远端粉碎性骨折的有效方法，手术配合至关重要．Objective The fracture at the distal end of femur can cause a complex damage of the bone and the tissue surrounding the bone. The conservative treatment always leads to the followings＂ mal-union, non-union and knee ankylosis. In addition, the incision is large, the bleeding is much, the tissue trauma is heavy, and the patients need more time to recover. In order to overcome the above prob lems, our hospital carries out the treatment of less invasive stabilization system （LISS）. This article is to introduce the experience of operative cooperation in the treatment of comminuted fracture at the distal end of femur with less invasive stabilization system （LISS）. Methods The operative cooperation for 5 cases with comminuted fracture at the distal end of femur was conducted, including visiting a patient before the operation~ putting the patient on an orthopedics operative tract table and drawing the patient＇s two legs for the fractures redaction; checking the results of redaction with C arm X-ray machine; conducting an internal fixation by providing the special apparatuses of LISS during the operation; and etc. Results The operations of all 5 cases were went smoothly. All patients were follow-up visited for 6 to 12 months （an average of 8 months）. The primary healing for the incision occurred in phase I, and a good position and alignment found in the plain X-ray film for all the cases. According to the scoring systems of Merchan for the function of the knee joint, 3 patients had an excellent result, and 2 patients had a fair result. Conclusions LISS is an effective method for treating the comminuted fracture at the distal end of femur. The operative cooperation is very important for conducting this operation successfully.
