四川省1991-2002年寄生虫病住院病例回顾性调查分析

摘　　要：目的为探讨寄生虫病在人群中的危害程度、发病的基本规律及特点,为临床诊断及拟订长期有效的防治措施提供科学依据。方法按照全国人体重要寄生虫病现状调查制定的寄生虫病例回顾性调查表,对住院病例的各项基本资料及入出院时间、诊断、转归及医疗费用等项分别进行查阅、记录。结果在24所医院中共查见寄生虫病住院病例3 311例,导致患者住院的寄生虫虫种有12种,其中单纯蛔虫感染病例占91.15%;钩虫感染占4.29%;血吸虫感染占3.23%,钩、蛔、鞭虫混合感染占0.39%。其它寄生虫感染占0.94%。寄生虫病住院患者病死率为0.15%。平均每位患者直接经济损失为1 239.77元。结论在寄生虫病住院病例中以蛔虫感染住院的病例数最多,其次是钩虫。在寄生虫病诊断过程中误诊率高达5.65%,加强寄生虫病健康教育,增强群众的防病意识和自我保护能力,提高基层临床医师的诊断水平,对于保护群众的身体健康有着积极意义。Objective To explore the species, damage and pathogenic characteristics of parasitoses and to provide scientific basis for clinic diagnosis and long-term control measures. Method All the case histories of parasitoses in-patients from 1991 to 2002 in 24 hospitals of Sichuan Province were screened and all the information listed into parasitosis case retrospective tables issued for national investigation on current situation of key human parasitoses. Result 3 113 parasitoses in-patients induced by 12 parasite species were recorded. Among them, single infection with roundworm accounted for 91.15% , with hookworm 4.29%, with schistosome 3.23% , while mixed infection with hookworm, roundworm and whipworm 0.39% and infection with other parasites 0.94%. The mortality rate of parasitoses in-patients was 0. 15%. The economic losses reached an average of 1 239.77 RMB yuan/case. Conclusion Ascariasis caused the highest rate of hospitalization and the next was acylostomiasis. The misdiagnosis rate reached as high as 5.65 %. It is necessary to enhance the human parasitosis diagnosis level among grass-roots doctors. Besides, the health education, self-protection and disease prevention consciousness among masses should be strengthened.
