带附外血管蒂骰骨瓣移位治疗距骨颈骨折脱位

摘　　要：目的探讨距骨颈骨折脱位的治疗方法。方法对10例距骨颈骨折合并脱位的患者行切开复位内固定,同时行带附外动脉血管蒂骰骨瓣移位植骨术治疗。根据Hawkins分型,Ⅱ型4例,Ⅲ型6例,全部病例均随访2年以上。结果优5例,良4例,可1例。未出现距骨不愈合及距骨缺血坏死,效果满意。结论笔者认为该方法有利于距骨血运重建,促进骨折愈合,能有效预防骨不连,骨坏死,是治疗距骨颈骨折脱位的较好选择。[Objective] To disscuss the therapy of the neck of the talus dislocation. [Methods] Using replacement innerfixaton to treat 10 cases patients who had fracture and dislocation of the neck of the talus. At the same time, the mehtods of adding artery pedicel and shifting cuboid lamella were used. According to Hawkins class, there were 4 cases of class and 6 cases of class. All the cases were followed above 2 years. [Results] The prognosis of 5 cases were evey good, 4 cases were good and 1 cases was ordinary. No case was no concrescence and putrescence. [Conclusion] We think this operation make for the rebuild of talus vessel, which can promot the concrescence of fracture. It can prevent the unconnection and putrescence of bone. To the cases of the neck of the talus dislocation, it is a good choice.
