摘  要:目的 探讨慢性乙型肝炎患者HBV-DNA在血清、唾液、尿液间可能存在的相关性及其流行病学意义。方法 采用荧光聚合酶链反应(FQ-PCR)精确定量配对检测200例慢性乙型肝炎患者血清、唾液、尿液中HBV-DNA含量。结果 200例慢性乙型肝炎患者中血清、唾液、尿液HBV-DNA检测阳性者分别为180例(90.0%)、14.5例(72.5%)、84例(42.0%)。HBV-DNA平均水平分别为血清6.51(2.69-10.00)log copies/ml,唾液4.52(2.69-7.54)logcopies/ml,尿液3.26(2.69-5.96)logcopies/ml。血清HBV-DNA与唾液、尿液HBV-DNA水平间均呈显著正相关(r分别为0.73,0.48,P〈0.001),唾液与尿液HBV-DNA之间也显著相关(r=0.73,P〈0.01)。结论 慢性乙型肝炎患者血液、唾液、尿液中含有感染性的高载量HBV-DNA,可成为传染源之一。血液中HBV-DNA含量与唾液、尿液中HBV-DNA含量有显著相关性,为慢性乙型肝炎的流行病学研究提供了新的思路和理念。Objective To explore if there are correlation between HBV - DNA levels in serum,saliva.urine in patients with chronic hepatitis B (CHB)and its significance on epidemiology. Methods Fluorescence quantitative PCR (FQ -PCR ) , which combines PCR with fluorescence probe hybridization ,was used to precisely determine HBV -DNA in serum, saliva, urine in 200 patients with CHB. Results HBV - DNA ( + ) rates of serum, salvia, and urine in 200 cases of CHB patients were 90% ,72. 5% , and 42% , respectively. Average HBV - DNA contents in serum, saliva and urine were 6. 51 ( 2. 69 - 10. 00 ) , 4. 52 ( 2. 69 -7.54 ) , and 3.26 ( 2. 69 - 5.96 ) log copies/ml. Strong positive correlations of HBV - DNA levels existed in between serum and saliva, urine (r=0. 73,0. 48 ; P〈 0. 001 ) ;saliva and urine ( r= 0. 73,P 〈0. 01 ). Conclusions Heavier loads of infectious HBV - DNA in serum,saliva,and urine have been found in CHB patients. This HBV - DNA could be one of the resources for hepatitis B infection. Positive correlation of HBV - DNA levels between serum, saliva, and urine may provide a new concept for the study of HBV epidemiology.
