摘  要:目的 评价关节镜下可吸收螺钉固定胭绳肌腱重建膝关节前交叉韧带(ACL)的效果。方法 27例患者(27膝)经关节镜检查证实均为ACL断裂,采用关节镜下自体胭绳肌重建ACL,生物可吸收螺钉解剖位固定重建韧带。结果所有病例均得到随访,时间6~24个月,均无术中、术后并发症,切口均一期愈合。IKDC综合评定:术前异常10例、严重异常17例,术后正常9例、接近正常16例、异常2例。Lysholm膝关节功能评分:由术前33—60分(平均45.7分)提高至83~96分(平均90.3分),差异有显著性(P〈0.05)。结论 关节镜下可吸收螺钉固定自体胭绳肌腱重建ACL手术创伤小、固定确实、操作简捷,近期疗效满意。Objective To evaluate the technique and outcome for arthroscopic reconstruction of anterior cruciate ligament with autogenous hamstring tendons fixed by bioabsorbable screw. Methods 27 knees with ACL tear in 27 patients were verified with arthroscopy in this department. All the cases were reconstructed with autogenous hamstring tendons under arthroscopy, and bioabsorbable screw was used for direct anatomic fixation of the reconstructed ligament. Results Follow-up time was ranged from 6 to 24 months in all 27 patients. No complication occurred during operation and after operation. Primary healing was achieved. In the IKDC assessment, IKDC score from abnormal in 10 knees and severely abnormal in 17 knees preoperatively to normal in 9 knees, nearly normal in 16 knees and abnormal in 2 knees. Lysholm knee scores were 33-60 points preoperatively in contrast to 83 - 96 points postoperative- ly with significant difference ( P 〈 0. 05 ). Conclusions Arthroscopic reconstruction of ACL with autogenous hamstring tendons fixed by bioabsorbable screw has advantages of not only minimal trauma in surgery and reliable fixation, but also simple operative procedure and satisfying effect in early stage postoperatively.
