摘 要:目的探讨反复支气管肺炎的患儿与HLA的相关性。方法采用序列特异性引物-聚合酶链反应(SSP/PCR)对86例反复患支气管肺炎小儿和81例随机挑选来院体检的健康儿童HLA-DRB1的19个等位基因进行分析,计算各个位点的基因分布频率和相对危险性RR值;并进行卡方检验,筛选有意义基因。结果反复支气管肺炎患儿HLA-DRB1*9和HLA-DRB1*12的分布频率明显高于健康儿童,观察组与对照组比较P<0.05,有显著性差异。其它各位的位点无显著性差异。疾病组与正常组之间进行逐个等位基因的比较,以相对危险性RR值计算,HLA-DRB1*9的RR值为2.46,HLA-DRB1*12的RR值为2.23。结论HLA-DRB1*9和HLA-DRB1*12可能是反复支气管肺炎小儿的易感基因。Objective To investigate the association of the children with recurrent bronchopneumonia with human leucocyte antigen system(HLA).Methods HLA-DRB1 19 alleles frequencies were measured in 86 children with recurrent bronchopneumonia and 81 healthy children using polymerase chain reaction with sequence specific primer, and calculate each allele 'frequencies and relative dangerousness RR value;and operate chi-sqare test,so that selecting significant genes. Results Allele frequency of HLA-DRB1 * 9 and HLA-DRB1 * 12 were significantly increased in the children with recurrent bronchopneumonia than that of healthy children,observasion group is compared with normal controls( P 〈 0.05), there was significant difference. There was no significant difference in other alleles.Every allele is compared between disease group and nomad group;and calculate relative dangerousness RR value,the RR value of HLA-DRB1 * 9 is 2.46, the RR value of HLA-DRB1 * 12 is 2.23.Conclusion HA-DRB1 *9 and HLA-DRB1 * 12 possibility were the predisposing genes of children with recurrent bronchopneumonia.
