摘 要:目的观察不同潮气量(VT)机械通气对大鼠肺组织Toll样受体-4(TLR-4)表达的影响。方法成年雄性SD大鼠30只,体重200~210 g,随机分为3组(n=10):自然呼吸组(C组)、小VT机械通气组(S组)和大VT机械通气组(L组)。S组VT8 ml/kg,L组VT40 ml/kg。机械通气3 h后放血处死大鼠,取肺组织,进行肺病理学评分,测定肺组织湿/干重比(W/D),对支气管肺泡灌洗液(BALF)中自细胞(WBC)进行计数,用RT-PCR法测定肺组织TLR-4 mRNA表达,用免疫组织化学法测定BALF中巨噬细胞TLR-4蛋白表达。结果与C组比较,L组肺组织病理学评分、W/D和BALF中WBC升高,肺组织TLR-4 mRNA及BALF中巨噬细胞TLR-4蛋白表达均升高(P<0.01),S组上述指标差异无统计学意义(P>0.05)。结论大VT机械通气3 h可上调肺组织TLR-4的表达。Objective To investigate the effect of mechanical ventilation with different tidal volumes on Toll-like receptor 4 (TLR-4) in rat lung tissue. Methods Thirty adult male SD rats weighing 200-210 g were randomly divided into 3 groups (n = 10 each): group Ⅰ (control) received no mechanical ventilation; group Ⅱ received mechanical ventilation with small tidal volume (VT = 8 ml·kg^-1 ) and group Ⅲ received mechanical ventilation with large tidal volume (VT = 40 ml· kg^-1). The animals were anesthetized with intraperitoneal 3 % pentobarbital 35 mg·kg^-1 , tracheostomized and mechanically ventilated ( I : E = 1 : l, PEEP = 0, FiO2= 21% ). The respiratory rate (RR) was adjusted to maintain the PET CO2 at 35-45 mm Hg. The rats were killed by exsanguination after 3 h mechanical ventilation. The fight lung was removed for determination of TLR-4 mRNA expression in lung tissue (by RT-PCR), W/D lung weight ratio, and microscopic examination. The left lung was lavaged and the expression of TLR-4 protein in macrophage collected from broncho-alveolar lavage fluid (BALF) was determined by immuno-histochemistry. Results The lung pathological scores, W/D lung w.eight ratio, the number of inflammatory cells in BALF, the expression of TLR-4 mRNA in lung tissue and TLR-4 protein in macrophage were significantly higher in large tidal volume group than in control group while there was no significant difference between small tidal volume and control group. Conclusion Mechanical ventilation with large tidal volume for 3h produces lung injury and up-regulation of TLR-4 expression in lung tissue.
