应用钢圈套扎吻合主、肺动脉进行心脏移植的实验研究

摘　　要：目的:评价钢圈套扎吻合主、肺动脉用于心脏移植的优缺点及其可行性。方法:应用传统吻合法和钢圈套扎吻合法模仿猪心脏移植时主、肺动脉的吻合,对比其主动脉和肺动脉吻合时间、吻合口出血发生率、动物存活率、血栓形成率、吻合口狭窄率等指标。结果:实验组10只动物全部存活,平均主动脉、肺动脉吻合时间为(12±3)min,无吻合口出血,术后3个月处死动物时吻合口无血栓形成,无吻合口狭窄发生。对照组8只动物全部存活,平均主动脉和肺动脉吻合时间为(44±16)min,吻合口出血发生率为12.5%(2/16),术后3个月处死动物时吻合口无血栓形成,无吻合口狭窄发生。结论:钢圈套扎吻合法用于心脏移植时的大动脉吻合,其方法简单易行,完成吻合所需时间短,效果与传统缝合法一样可靠,可明显缩短心脏移植手术中两大动脉吻合时间,从而更有利于心肌保护,具有很好的临床应用前景。Objective To evaluate the advantage, disadvantage and the possibility of the pulmonary and aortic arteries anastomosis by using the self-made stainless steel endovascular stent（SSE stent）for heart transplantation. Methods The pig heart transplantation was done by using traditional suture anastomosis or SSE stent ligating anastomosis, respectively. The pulmonary and aortic arteries anastomosis time, anastomosis heroorrhage, anastomosis stenosis and thrombosis,and the survival rate of the pigs were compared. Results All the 10 pigs were survival from the transplantation in experiment group. The pulmonary and aortic anastomosis time is （12 ± 3） min. There were no anastomosis hemorrhage, stenosis and thrombosis when the pigs were killed at 3 months past operation. All the 8 pigs with traditional suture anastomosis were also survival from the heart transplantation. The pulmonary and aortic arteries anastomosis time was （44 ± 16）min. There was 12.5% （2/16） incidence of bleeding. There was neither stenosis nor thrombosis. Conclusion The SSE stent ligating anastomosis can be safely used for pulmonary and aortic arteries anastomosis in heart transplantation.
