摘 要:【目的】评价1,5-脱水山梨醇(1,5-AG)在糖尿病诊治监测中的临床意义。【方法】对42例非糖尿病对照、38例糖耐量减退和84例2型糖尿病患者进行研究。对照组测定空腹血糖和1,5-AG,糖耐量减退组测定空腹血糖(FBG)及餐后2 h血糖(2 hBG)和1,5-AG;2型糖尿病患者测定FBG及2hBG、果糖胺(FMN)、糖化血红蛋白(HbA1c)和1,5-AG。比较三组1,5-AG的差别,并以1,5-AG为自变量,FBG、2hBG、FMN和HbA1c为因变量行回归分析。【结果】糖耐量减退组1,5-AG水平明显低于非糖尿病对照组[(107.9±43.5)μmol/L对(149.9±46.5)μmol/L,P<0.01],但与年龄、FBG和2 hBG不相关;糖尿病组1,5-AG水平明显低于非糖尿病对照组[(46.3±38.7)μmol/L对(149.9±46.5)μmol/L,P<0.01],且与FBG、2hBG、FMN和HbA1c呈负相关。【结论】1,5-AG仅反映高血糖状态,是一种新的糖代谢指标,与FMN、HbA1c等指标结合,更有利于糖尿病的监测。[Objective]To investigate the clinical significance of 1,5-anhydro-D-glucitol(1,5-AG) in monitoring the process of diagnosis and treatment of diabetes mellitus. [Methods]Forty two non-diabetic controls, 38 impaired glucose tolerance (IGT) and 84 type 2 diabetes mellitus patients were enrolled into the study. Fasting blood glucose and 1,5-AG were measured. Extra postprandial serum glucose was measured in IGT. Additionally, postprandial serum glucose, Fruetosamine (FMN) and Glyeosylated A1 (HbAle) were tested in the type 2 diabetes mellitus group. The difference among three groups was compared. Meanwhile, 1,5-AG independent variable and FBG,2 hBG,FMN,HbAle dependent variable were made respectively,and to do linear correlation analysis. [Results]Serum level of 1,5-AG was obviously lower in IGT than that in the non-diabetes control[(107.9±43.5) /μmol/L vs (149.9±46.5) /μmol/L, P 〈0.01] and it was not correlated with Age. FBG and 2hBG levels. Serum level of 1,5-AG was obviously lower in the diabetes than that in the non-diabetes control[(46.35±38.8) μmol/L vs (149.9±46.5) μmol/L, P〈0. 01] and it was negatively correlated with FBG,2hBG FMN and HbAle levels. [Conclusion]As a new marker for the glucose metabolism, 1,5-AG is affected by the hyperglycemia only. In combination with other markers, such as FMN and HbAle, serum level of 1,5-AG is very helpful in monitoring the process of diagnosis and treatment of diabetes mellitus.
