老年股骨颈骨折和粗隆间骨折患者的围手术期处理

摘　　要：目的探讨老年股骨颈骨折和粗隆间骨折患者的围手术期处理。方法回顾性分析33例老年股骨颈骨折和粗隆间骨折患者围手术期资料。结果股骨颈骨折行人工双极股骨头置换,粗隆间骨折行动力髋固定。本组术前30例有内科合并症,其中心脑血管系统疾患最常见。术后8例发生并发症,其中神经精神系统3例,心血管系统3例,呼吸系统1例,尿路感染1例。住院期间无死亡。结论积极的围手术期评估和处理是老年股骨颈和粗隆间骨折患者手术治疗成功的主要措施之一。Objective To evaluate perioperative treatment of elderly patients with femoral neck or intertrochanteric fracture. Methods Data of 33 elderly patients with femoral neck or intertrochanteric fracture were retrospectively analyzed. Patients were treated with replacement of artificial femoral head or DHS fixation. Results 30 cases with internal medical disease before operation, in which cerebrovascular diseases were common. Complications happened in 8 case after operation involving 3 cases nervous system, 3 cardiovascular system, 1 respiratory system and 1 urinary tract infection. No mortality occurred in hospital. Conclusions The positive results can be achieved by overall evaluation and intensive treatment during perioperation on the elderly patients with femoral neck or intertrochanteric fracture.
