摘 要:目的探讨铁路职工卫生服务需求特点,为制定医院管理相关对策提供依据。方法由课题小组自行设计调查问卷,采用整群随机抽样方法,对成都地区铁路工程总公司、客运段、机务段、工务大修队、铁路公安局等13个铁路单位3104位职工进行问卷调查。结果(1)30天患急病/伤率11.53%。5种慢性病患病率为13.50%。半年内因病/伤休工率6.12%、卧床率3.87%。(2)半年内因病伤到医疗机构诊治者占52.19%。(3)铁路医改后就医首选医院以地方医院居多(门诊53.67%,住院55.41%);仍选铁路中心医院者居第二位(门诊33.44%,住院36.53%)。结论面对医疗保障制度改革,铁路医院要主动适应并积极应对医疗市场的竞争,加强科学管理,规范医疗行为,更好地为社会提供公平、合理、有效的卫生服务。Objective In order to establish the related strategies and provide evidence for the administration of the hospital, the paper has made an analysis of the characteristics of the need of hygiene and service of the railway staff and workers. Method 3104 staff and workers from 13 different railways units such as the engineering controlling corporation, passenger transportation depot, locomotive depot, overhaul-repairing depot and public security bureau have been investigated under the questionnaire designed by the study group in the way of random sampling in groups. Results 1. Those who suffer from acute diseases and wounds are 11.53 % within a month. Those who suffer from five kinds of chronic diseases are 13.50%. Those who suspend their working because of various diseases and wounds within half a year are 6.12%. Those who confine to bed are 3.87%. 2. Those who receive the treatment because of diseases and wounds within half a year are 52.19%. 3. Railway staff and workers are first sent to the local hospitals after the railway medical reform ( the rate of out-patients is 53.67 while the rate of in-patients is 55.41 ). The hospitals in Chengdu are first chosen. Railway Central Hospital for treatment is taken the second place ( the rate of out-patients is 33.44 while the rate of in-patients is 36.53 ). Conclusion Facing with the medical security reform, it is of vital importance for the railway hospital to adapt to deal with the fierce competition of the medical market. At the same time, it must tight up the management scientifically, standardize the behavior of the staff of the hospital and provide fairer, more reasonable and effective service for the society.
