摘  要:目的观察并探讨VEGF、bFGF在退变椎间盘组织中的表达的意义及其与新生血管的关系。方法将58例来源于腰椎间盘突出症患者手术中所取得的椎间盘组织,与12个取自腰椎骨折脱位行前路手术者(6例)的正常椎间盘(L1~5)组织进行对比。通过免疫组织化学方法,用CD34标记血管内皮细胞并检测各自椎间盘组织中的VEGF、bFGF的表达。结果年龄与CD34、VEGF、bFGF表达阳性率无关(P〉0.05)。病程超过1年者CD34、VEGF阳性率明显低于1年以内者(P〈0.05),bFGF阳性率与病程无关。游离型与突出型CD34、VEGF阳性表达率高于隆起型(P〈0.05),bFGF阳性率在3种突出类型间无明显差异(P〉0.05)。CD34、VEGF和bFGF阳性率两两相关(P〈0.05)。结论VEGF与bFGF参与了退变椎间盘新生血管的形成过程并可能有协同作用,提示VEGF在新生血管形成过程中的启动作用,而bFGF的作用则贯穿椎间盘退变整个过程。Objective To observe angiogenesis and the expression of vascular endothelial growth factor and basic fibroblast growth factor and discuss its significance in normal and degenerated lumbar intervertebral discs. Methods Fifty-eight tissue samples surgically removed from 58 patients with herniated discs and 12 tissue samples (L1 - 5) from 6 persons with lumbar fracture anterior release were studied. In the immunohistochemical study,an anti-CD34 antibody for vascular endothehal "cells, the expression of VEGF and bFGF were tested by immunohistochemistry techniques. Results There was no direct relationship between the patient's age and the positive rate of expression of CD34, VEGF and bFGF ( P 〉 0.05). The positive rate of expression of CD34 and VEGF in those with a shorter course of illness of less than one year was higher than that of those whose course longer than one year ( P 〈 0.05). Patient's course of illness did not relate to bFGF expression. The positive rate of expression of CD34 and VEGF in extrusion type and sequestration type was higher than protruded type ( P 〈 0.05). The type did not relate to bFGF expression ( P 〉 0.05). A positive correlation between the positive rate of expression of VEGF and bFGF ( P 〈 0.05)was observed. Condusion Thus capillaries in disc herniation tissue are evidently rebirthed. These results demonstrate that VEGF and bFGF participate in the neovascularization process and may cooperate each other. The results also suggest that VEGF may play a key initial role in neovascularization process, bFGF may contribute to full process of intervertebral disc degeneration.
