摘 要:目的探讨过敏性紫癜患者血清中过敏原特异性IgE抗体和血清总IgE水平,了解过敏性紫癜过敏原的组成,为临床寻找病因和做好有效的预防提供实验室依据。方法采用酶联免疫分析法对120例过敏性紫癜患者血清过敏原特异性IgE抗体及血清总IgE水平进行检测。结果120例患儿中血清总IgE阳性者为112例,占93.3%,血清过敏原特异性IgE抗体阳性103例,占85.8%;其中对一种过敏原反应阳性者42例(35%),对两种以上过敏原反应阳性者61例(50.8%);吸入组中特异性IgE最常见的致敏原是律草(15%),其次是多价霉菌(11.7%)、尘螨/粉螨(11.7%);蛋白食物组中以虾/蟹(15.8%)为最高,其次是鱼(10.8%)和蛋类(9.2%);蔬菜水果食物组以西红柿/菠菜/芹菜(14.2%)为最高,其次是大豆/胡萝卜/南瓜(10%)和生菜/莴苣/蘑菇/黄瓜(10%)。结论过敏性紫癜患者血清过敏原混合过敏较多,需针对相应过敏原做好有效预防,进行有效治疗。提供实验室诊断和防治依据。Objective To detect serum levels of total IgE and specific IgE of patients with allergic purpura, and to investigate the components of the allergen, so as to provide laboratory basis for searching for cause and effective prevention of the disease. Methods 120 patients were examined for serum concentrations of total IgE and specific IgE by using EMSA. Results The saturn total IgE was positive (93.3%) in 112/120 and the serum allergen-specific IgE was positive in 103/120(85.5% ). Of which, positive reaction to one kind of allergen was found in 42 cases (35%) and that to two or more than two allergens was found in 61 cases (50. 8% ) ; the most common allergen for specific IgE in inhalation allergen group was Herha Humuli ( 15% ), the next was polyvalent fungi (mycete) ( 11.7% ) and dust mite/food mite. In protein food allergen group, the most commom allergen was shrimp/crab(15.8), and the next was fish ( 10.8% ) and egg (9.2%). In vegetalble and fruit group, the most commom allergen was tomato/spinach/celery ( 14.2% ), the next was beans/ carrot/pumpkin and salad/lettuce/mushroom/cucumber ( 10% ). Conclusion By detecting serum allergens of patients with allergic purpura using EMSA, it was found that in the positive rate of serum allergen, there was no significant difference, between inhalation group and food group,and the combined allergens were more common, which can provide a laboratory basis for diagnosis, effective prevention and treatment of allergic purpura according to allergens found.
