高压氧治疗对原发性脑干损伤患者血糖的影响和疗效分析

摘  要:目的:探讨高压氧(HBO)治疗与原发性脑干损伤(PBS I)患者血糖和预后的关系。方法:64例PBS I患者分为HBO治疗组(HBO组)和常规治疗组(对照组)。测定两组患者治疗10d前后血糖浓度。按GO S标准判断疗效。结果:治疗后两组患者血糖均显著下降(P<0.01),且HBO组下降较对照组明显(P<0.05)。HBO组患者平均昏迷时间39 d,死亡8例,植物生存1例。对照组平均昏迷时间52 d,死亡15例,植物生存2例。HBO组疗效显著高于对照组(χ2=4.14,P<0.05)。结论:PBS I患者血糖显著升高,HBO能够降低PBS I患者升高的血糖,对PBS I的治疗有效。Objective :To explore the relationship between hyperbaric oxygen treatment, plasma glucose level and prognosis in the patients with primary brain stem injury (PBSI). Methods:Sixty--four patients with PBSI were divided into hyperbaric oxygen treated group (HBO group) and routine treated group (control group). In all patients, the plasma glucose level was measured before and after 10- day treatment. The neurological prognosis was evaluated with glasgow outcome scale (GOS). Results :After 10 --day treatment, the plasma glucose levels of two group were significantly lower than those before treatment respectively (P 〈0.01) ,and the plasma glucose value in HBO group was significantly decreased compared with controlgroup (P〈0.05).In HBO group, the average time of stupor was 39 days,8 cases died and 1 case alive as a persistent vegetative state (PVS), while in control group, the average time of stupor was 52 days, 15 cases died and 2 cases alive as a PVS. The treatment efficacy in HBO group was significantly higher than that in control group (X^2= 4.14,P 〈0.05) . Conclusion:The Patients with PBSI frequently develop hyperglycemia. Hyperbaric oxygen treatment may decrease plasma glucose in patients with PBSI and is an effective approach for patients with PBSI.
