摘 要:目的观测及探讨腹腔镜手术对婴幼儿呼吸循环的影响。方法择期行腹腔镜手术的患儿50例,ASAⅠ或Ⅱ级,选用气管内插管静吸复合全麻。异丙酚麻醉诱导插管后行机械通气。记录术前至术后不同时点的血压、心率、脉搏氧饱合度(SPO2),呼气末CO2浓度(PetCO2)和气道压(peak)值。结果与气腹前相比,CO2气腹15分钟后,SBP,MAP和HR显著升高(P<0.01);peak值和PetCO2显著升高(P<0.01)。血气分析显示,血液pH值在CO2气腹后明显降低(P<0.01),呈酸血症。结论小儿腹腔镜手术对患儿呼吸循环影响较大,应加强术中监测,并积极做出相应处理。Objective To observe and investigate the effects of infant respiration and circulation in laparoscopic surgery. Method Fifty children, ASA Ⅰ ~Ⅱ,scheduled for laparoscopic surgery were selected. The intubation and combined vein-inhale general anesthesia was adopted. Mechanical ventilation was used after propofol induction. BP, HR, SPO2, PetCO2 and Peak were determined at the four time points: before inducing, 5min after intubation ,15min after aeroperitoneum and 5min after surgery. Results SBP, MAP and HR increased significantly 15min after aeroperitoneum (P〈0.01),also PetCO2 and Peak. But the blood PH was significantly lower after aeroperitoneum than before induction (P〈 0.01),as acidemia. Conclusion There were great effects to infant respiration and circulation in laparoscopic surgery.
