两种方法治疗慢性分泌性中耳炎的疗效比较

摘　　要：目的探讨鼻内镜下行咽鼓管置管术治疗慢性分泌性中耳炎的疗效,并观察和分析咽鼓管咽口形态与分泌性中耳炎的关系。方法将慢性分泌性中耳炎患者73例(88耳)随机分成置管组和对照组,分别行鼻内镜下的咽鼓管置管术和传统的咽鼓管吹张术,术后随访3个月,比较两组疗效。结果置管组38例(45耳):治愈17耳,占37.8%,好转21耳,占46.6%,总有效率84.4%;对照组35例(43耳):治愈8耳,占18.6%,好转22耳,占51.2%,总有效率69.8%。治疗后两组治愈率差异有显著性意义(P<0.05)。结论鼻内镜下咽鼓管置管术具有微创、安全、简单、疗效好等特点,是治疗慢性分泌性中耳炎的一个良好方法;咽鼓管咽口形态可分五种类型,与分泌性中耳炎有着密切的联系。Objective To evaluate the curative effect of the treatment that eustachian tube insertion under nasal endoscope in treating the secretory otitis media, and to analysis the relation between the pharynx orifice of the eustachian tube and the secretory otitis media. Method There were 73 patients （88 ears） which were randomly allocated into two groups：the insertion group and the control group were each treated by different methods： eustachian tube insertion under nasal endoscope and eustachian tube blowing. All patients were followed up by the end of the third months after operation, and the effect was evaluated. Result The insertion group：17ears （37.8%） were cured, 21 ears （46.6%） were straightenedup, The total efficiency rate was 84.4%. The control group： 8 ears （18.6%） were cured, 22 ears （51. 2%） were straightened up, the total efficiency rate was 89.8%. There are significant difference between two groups after treatment （P〈0.05）. Conclusion Eustachian tube insertion under nasal endoscope is simple and safe, and it has a tiny wound and a good curative effect. It is a good way for treating the secretory otitis media. The pharynx orifice of the eustachian tube can be separated into five types, and it has consanguineous relations to the secretory otitis media.
