麻醉综合疗法治疗粘连性肩周炎18例

摘　　要：目的探讨麻醉综合疗法在治疗粘连性肩周炎的作用。方法18例确诊粘连性肩周炎的患者,痛点注射消炎镇痛液+小针刀松解剥离,每周1次,两周后继以痛点+颈5横突注射,在异丙酚静脉全麻下行手法松解,术后行以盐酸曲马多注射液为主的静脉病人自控镇痛(PCIA),辅以口服适量非甾体消炎镇痛药(NSAIDs)、神经营养药和镇静药等,配合运动疗法进行功能锻炼。结果18例均取得了满意的疗效,随访3～6个月未见复发。结论该麻醉综合疗法治疗粘连性肩周炎是一种疗效确切,见效快,安全,患者痛苦小,易于接受的治疗方法。Objective To investigate the effect of anaesthesia synthesis therapy at treatment adhesion periarthritis shoulder . Methods 18 cases diagnose the adhesion periarthritis of shoulder patients, pain spots injection with sterilization analgesia fluid and acupotomy, each week 1 time, two weeks later by the pain spots injection with sterilization analgesia fluid, and the fifth processus transversus of cervical vertebra injection with 1% Lidocaine 10ml（content Methycobal 0.5mg） ,and technique loose the adhesion bursitis in shoulder under propoful vein general anesthesia, postoperative analgesic by patient-controlled intravenous analgesia（PCIA）,the main analgesics is Tramadol. At the same time, let the patients take orally right amount drug of nonsteroidal anti inflammatory durugs（NSAIDs）, the nerve nutrients and the sedative, and so on. Coordination kinesiotherapy carries on the function exercise. Result 18 cases have obtained the satisfactory curative effect, makes a follow-up visit 3-6 months not to find recurs. Conclusion This anaesthesia synthesis therapy treatment adhesion periarthritis shoulder is one kind of curative effect accurate, effect quick, safe, the patient pain little and willing to accept method of treatment.
