摘 要:目的观察囊型肝包虫周围肝细胞的病理形态学变化(肝细胞萎缩、坏死、凋亡),初步探讨囊型肝包虫病肝细胞“消失”机制。方法对30例肝包虫囊肿周围肝组织通过光镜观察肝组织的病理形态学变化,运用TUNEL法测定肝细胞凋亡,采用免疫组化技术检测肝包虫囊肿周围肝组织及正常肝组织中Bcl-2及Bax蛋白的表达。结果TUNEL检测囊型肝包虫病患者肝细胞凋亡指数(TI)为0.12%,与正常肝组织(TI=0.16%)比较差异无显著性(P>0.05);Bcl-2及Bax蛋白的囊周肝组织呈低表达,分别为6.67%和13.33%,正常肝组织Bcl-2和Bax均为10.00%,差异无显著性(P>0.05)。病理组织学观察示肝细胞萎缩,肝细胞坏死明显。结论肝细胞萎缩、坏死可能是引起肝细胞“消失”的主要机制,肝细胞压迫性和营养不良性萎缩、肝细胞坏死、肝细胞凋亡共同参与囊型肝包虫病肝细胞“消失”过程。Objective To observe the pathologic changes (atrophy, necrosis, apoptosls ) of perlcystic bepatorytes in hepatic hydatid disease, so as to study the mechanism of hepatocyte "disappearance" on hepatic cystic echinococcosis. Methods To observe the pathologic changes of hepatocytesby light microscope and to detect the hepatocyte apoptosis by the terminal deoxynucleotide transferase-mediated deoxyuridine triphosphate nick end-labeling (TUNEL) method. Meanwhile, the expression of Bcl-2, Bax protein was detected by immunohistochemical method in 30 cases of the pericystic tissues of hepatic hydatid, 10 cases of normal hepatic tissues were assigned to control group. Results The results of TUNEL assay showed that no significant difference was found between TI of hepatocytes (TI=0.12 % ) in the pericystic hepatic tissues and in normal liver(TI =0.16%). The expression of Bcl-2 and Bax protein was 6.67% and 13.33% repectively, both expression in normal liver tissue were 10.00%, No statistical difference was observed between the positive rate of Bcl-2 and Bax in hepatic cystic echinococcosis and in normal liver. The pathological obversation indicated that hepatocyte atrophy and necrosis could be seen obviously. Conclusion The results indicate that hepatocyte atrophy and necrosis are possibly a predominant mechanisms to hepatocyte "disappearance" in hepatic cystic echinococcosis, Hepatocyte pressure atrophy, malnutrition atrophy, necrosis and live apoptosis coparticpiate in the process of "disappearance " of hepatocyte in hepatic cystic echinococcosis.
