30°会阴切开及皮内缝合术的临床效果研究

摘　　要：目的探讨不同会阴切开方式及缝合技术的临床效果。方法随机选择326例来我院经阴道分娩的足月初产妇,按双盲原则分为两组,实验组(166例)行会阴30°切开及薇荞线皮内包埋缝合术,对照组(160例)行会阴45°侧切开及羊肠线、丝线缝合术,从会阴切口撕裂伤率、出血量,术后伤口肿胀、疼痛、愈合、住院时间进行研究比较。结果观察组阴道裂伤率31.38%,对照组阴道裂伤率为57.5%,经统计差异有显著性意义(P<0.001);且观察组会阴切口出血少、疼痛轻、住院时间短、切口愈合良好,与对照组比较差异有显著性意义(P<0.05,P<0.001)。结论改良式30°会阴切开及皮内缝合术其对产妇会阴损伤小,切口肿胀疼痛反应轻,切口愈合良好、住院时间短,有利于产妇早下床活动,对提高母乳喂养率及产科质量有独特的意义。Objective To investigate the clinical effect on the different perineum incision ways and saturation techniques, Method 326 bospitalized primiparae were randomly divided into two groups. 130 cases as observing group and 122 case as control group, The ways of 30° perineum incision way and hypodermic saturation technique was used in the observing group and 450 perineum incision way and the saturation technique using absorbable catgut suture or silk thread was used in comrol group. The clinical effect on the cut laceration, bleeding, swelling, ache, healing, infection and the time in the hospital was compared between the two group, Result The rate of the vaginal laceration was 32.30% in the observing group, but that was 57.38% in the control group. There was significant difference （P 〈（0.001）, By the postpartum, nursing observation, there were significant differences in the vaginal cut healing and the comfortable feeling between the two groups （P〈0. 05）. Conclusion The improved 30° perineum incision way and the hypodermic saturation technique were better, This way have many advantages, such as the smaller hurt, the light reaction of the swelling and ache, the beautiful, cut after healing, the short time in hospital, early freedom activity out of the bed, in benefit of breast-feeding and improving obstetric quality.
