摘  要:目的总结浙江省首例同种异体小肠移植术围手术期的血流动力学和生化变化及其意义。方法术中密切监测血流动力学参数,根据手术刺激强度、进程和血流动力学和生化变化调节麻醉深度,维持有效循环血量、电解质和酸碱平衡。结果供体小肠修整灌注顺利,受体手术过程顺利,持续8.5h,术中血流动力学维持稳定,但有持续的低钾血症。术毕清醒后拔除气管导管,术后34天因移植物抗宿主病死于多脏器功能衰竭。结论维持小肠移植术中的血流动力学稳定和基本正常的生化指标是保证手术成功和术后顺利恢复的重要一环。Objective This paper is to review the hemodynamic change for the first case of isolated segmented small bowel transplantation in Zhejiang. Methods During the operation, hemodynamic parameters were closely monitored. The depth of anesthesia was adjusted based on the surgical stimulation, course and hemodynamic changes. The blood volume, electrolyte balance and acid - base equilibrium were maintained. Results The surgery of both the donor and the recipient went smoothly, and the recipient operation lasted for 8.5h. The vital signs were maintained stable during the operation but hypokalium. The patient was extubated after emergence from anesthesia postoperatively, but died of multiple organ failure because of the bowel graft to induce a graft - versus - host disease (GVHD). Conclusions It's important to sustain the stable of hemodynamic and normal biochemical result for a successful operation of small bowel transplantation.
