中性粒细胞碱性磷酸酶在骨髓增生性疾病诊断及疗效观察中的价值

摘　　要：目的探讨中性粒细胞碱性磷酸酶(NAP)活性在骨髓增生性疾病(MPD)诊断、鉴别诊断、疗效观察及预后判断中的临床价值。方法MPD患者106例,其中慢性粒细胞白血病(CML)38例、骨髓纤维化(MF)18例、原发性血小板增多症(ET)20例、真性红细胞增多症(PV)23例、慢性中性粒细胞白血病(CNL)7例。取健康体检者108例为对照。静脉血用Kaplow偶氮耦联法进行NAP染色,同时检测正常体检者及MPD患者外周血白细胞,MPD患者治疗前后骨髓穿刺及骨髓活检。结果正常体检者NAP阳性率及其积分中位数分别为30.2%,39.8;而MPD患者组中,CML,MF,ET,PV及CNL的NAP阳性率及积分中位数依次为5.7%,8.2;63.4%,128.1;72.5%,161.5;75.8%,182.2;98.1%,302.4。治疗2￣4个疗程后,NAP阳性率及积分中位数依次为18.2%,32.3;59.6%,102.4;43.2%,88.6;46.4%,89.6;66.4%,126.3,治疗前后两者差异具有统计学意义(P<0.01)。结论NAP检查对MPD的诊断、鉴别诊断、治疗以及缓解与复发的监测有一定参考价值。Objective To discuss the clinical value of NAP in patients with MPD and normal controis. Methods We collected vein blood of normal controls （n =108） and MPD patients （n=106, including 38 cases of CML, 18 cases of MF, 20 cases of ET, 23 cases of PV and 7 cases of CNL）, prepared blood smear and examined NAP by Kaplow NAP staining method. At the same time, the number of WBC was detected and the puncture and biopsy of bone marrow were performed in patients with MPD. Results The mean percent of positive and integral score of NAP of normal controls was 30.2 %, 39.8 respectively. However, in the group of MPD, the average percent and score in CML, MF, ET, PV, CNL was 5.7 %, 8.2; 63.4 %, 128.1; 72.5 %, 161.5; 75.8 %,182.2; 98.1%, 302.4 respectively. After two or four periods of treatment, the average percent and score was 18.2 %, 32.3; 59.6 %, 102.4; 43.2 %, 88.6; 46.4 %, 89.6; 66.4 %, 126.3 respectively. There were significant difference between these two groups（P 〈0.01）. Conclusion The detect of NAP is of clinical value in the diagnosis, differentiate diagnosis and inspect the recurrence and catabasis of these patients.
