红花注射液治疗慢性肺源性心脏病急性加重期的疗效及机制

摘　　要：目的：观测红花注射液在慢性肺源性心脏病急性加重期的临床疗效,探讨其机理。方法：选择78例慢性肺源性心脏病急性加重期的患者,随机分成两组,试验前均评价心功能及监测动脉血气、右室压(RVP)、平均肺动脉压(mPAP)、全血高切粘度、全血低切粘度、血浆粘度、红细胞压积、纤维蛋白原、血红蛋白定量和红细胞计数等指标。然后,红花组在给予常规治疗的同时合并给予红花注射液20ml/d,稀释后静脉点滴,疗程2周；对照组只给予常规治疗。2周后,复测上述指标,比较两组治疗前后的变化。结果：(1)红花组心力衰竭纠正动脉血气改善的有效率显著高于对照组(P＜0．05)。(2)红花组治疗前后比较,RVP、mPAP均明显下降(P＜0．05)；而对照组无显著变化(P＞0．05)。(3)红花组治疗前后比较,全血高切粘度、全血低切粘度、血浆粘度、红细胞压积、纤维蛋白原、血红蛋白定量和红细胞计数等指标均明显下降(P＜0．05)；而对照组则无显著变化(P＞0．05)。结论：红花注射液治疗慢性肺源性心脏病急性加重期具有纠正心力衰竭、降低肺动脉和右心室压力、改善血液流变和动脉血气的作用。Objectve..To investigate the clinical efficacy of Safflower Injection on acute exacerbation of chronic cor pulmonale and explore the principle. Methods： 78 acute exacerbation chronic cor pulmonale patients were randomly assigned into two groups, all the patients were measured the cardiac function and the levels of arteria blood gases, right ventricle pressure,average pulmonary artery pressure, blood viscosity, plasma viscosity, hematocrit （PCV）, fibrinogen quantity, ferrohemoglobin quantity and RBC before the trial; In addition to the routine treatment, the Safflower group patients were given Safflower Injection,intravenous drip 20 ml/d for 2 weeks, but the control group were only given routine treatment. 2 weeks later, all markers were measured again and compare the change of them. Results.. （1） Heart failure redressing rate and arteria blood gases ameliorating rate of Safflower group were higher than control group（P〈0. 05）. （2）2 weeks later, the markers of right ventricle pressure, average pulmonary artery pressure, blood viscosity, plasma viscosity, hematocrit, fibrinogen quantity, ferrohemoglobin quantity, RBC in Safflower group were decreased （P〈0. 05 ）, but there were no change in control group （P〉0.05）. Conclusion：safflower Injection can redress heart failure, decrease the pressure of right ventricle and pulmonary artery and amel- iorate arteria blood gases and hemorheology.
