摘  要:目的:探讨倍他乐克防治急性心肌梗死(AMI)后胶原重构的作用机制。方法:将AMI模型24例随机分成倍他乐克组和AMI组,另设8例假手术组,试验结束时,存活动物分别为倍他乐克组8例,AMI组10例,假手术组7例。所有新西兰兔分别于术前、术后1、4 d检测血清白介素-6(IL-6)、于术前、术后17、d检测超敏C反应蛋白(hs-CRP)及术前、术后4、71、42、8、56 d血清Ⅰ型前胶原羧基端肽(PⅠCP)和Ⅲ型前胶原氨基端肽(PⅢNP)的含量,所有指标均用酶联免疫分析(ELISA)方法检测。56 d时处死试验动物,取梗死区及非梗死区组织,用免疫组化方法检测梗死区和非梗死区的胶原沉积。结果:术后各组各时段兔血清PⅠCP和PⅢNP浓度均升高,倍他乐克组和AMI组明显高于假手术组(P<0.01),但倍他乐克组低于AMI组(P<0.05),且持续56 d以上。倍他乐克组和AMI组较假手术组梗死区及非梗死区胶原沉积和Ⅰ/Ⅲ型胶原比值明显增加(P<0.01),倍他乐克组梗死区及非梗死区胶原沉积较AMI组减轻(P<0.05);倍他乐克组非梗死区Ⅰ/Ⅲ型胶原比值较AMI组减低(P<0.05),梗死区无明显差异(P>0.05)。倍他乐克组和AMI组术后4 d血清IL-6明显高于假手术组(P<0.01),倍他乐克组低于AMI组(P<0.05);倍他乐克组和AMI组术后7 d hs-CRP明显高于假手术组(P<0.01),但倍他乐克组低于AMI组(P<0.05)。PⅠCP峰值与梗死区、非梗死区Ⅰ型胶原沉积呈正相关,PⅢNP峰值与Ⅲ型胶原沉积呈正相关;术后4 d血清IL-6和术后7 d血清hs-CRP与PⅠCP和PⅢNP峰值呈正相关性。结论:AMI后应用倍他乐克可减轻胶原沉积,其抗胶原重构作用有一部分是通过抑制炎症反应实现的。Objective: To investigate the precautionary and therapeutic action of Metoprolol on collagen remodeling after acute myocardial infarction(AMI) in New Zealand rabbits. Methods: MI was established by ligating the left ventricular branch of coronary artery in 24 New Zealand rabbits who were randomly divided into 2 groups, Metoprolol group and AMI group. And the other 8 rabbits were enrolled into sham operation group. After test, 8 rabbits survived in Metoprolol group, 10 in AMI group, and 7 in sham operation group. The serum level of IL-6 was measured before operation, 1 day and 4 days after operation, and the level of hs-CRP before operation, 1 day and 7 days after operation. The serum levels of PⅠCP and were masured before operation and at 1, 4,7, 14, 28, 56 days after operation in all New Zealand rabbits. All indexes were measured by ELISA. After the experimental animals were executed at 56 days, pieces of myocardium in farction zone and in uninfarction zone were excised, and the deposition content of collagen Ⅰ and Ⅲ were measured by immunohistochemistry method. Results: The levels of PⅠCP and PⅢNP were enhanced in three groups, but were higher in Metoprolol and AMI groups than in sham group and were lower in Metoprolol group than in AMI group, which lasted more than 56 days. The deposition contents of collagen Ⅰ , Ⅲ and ratios of collagen Ⅰ ,Ⅲ increased not only in infarction zone but also in tminfarction zone in Metoprolol and AMI groups. The deposition contents of collagen Ⅰ , Ⅲ were reduced in infarction zone and in tminfarction zone in Metoprolol group compared with those in AMI group. The ratios of collagen Ⅰ , Ⅲ were relieved in tminfarction zone, but were not in infarction zone in Metoprolol group. The level of IL- 6 at 4 days was higher in Metoprolol and AMI groups than in sham operation group, was lower in Metoprolol group than in AMI group. The level of hs-CRP at 7 days was higher in Metoprolol and AMI groups than in sham group, was lower in Metoprolol group than in
