摘  要:目的:探讨高频超声对原发性甲状旁腺功能亢进症(PHPT)术前定位的作用,并与99mTc-MIBI显像法比较。方法:在71例术前疑诊PHPT患者中进行超声检查68例,99mTc-MIBI核素显像检查55例,2项影像学检查均进行的患者53例,将检查结果与手术及病理结果作比较。结果:高频超声和99mTc-MIBI显像的灵敏度分别为78.5%和90.6%,阳性预测值分别为94.4%和98.0%。当联合运用超声和核素检查后,灵敏度达到96%。结论:对PHPT患者,高频超声检查和,99mTc-MIBI显像都能较准确地定位诊断,两方法联合运用,则能提高对肿大甲状旁腺的检出率。Objective To assess the value of high frequence ultrasound (US) in preoperative localization of parathyroid glands in primary hyperparathyroidism, and to compare with ^99mTechnetium sestamibi scintigraphy(^99mTc-MIBI). Methods The preoperative localization was performed in 68 cases using US, in 55 cases using ^99Tc-MIBI, and in 53 patients using both the imaging methods. The results were compared with the operative and histologic findings. Results The sensitivity and positive predictive value was 78.5% and 94.4% for US, and 90.6% and 98.0% for ^99mTc-MIBI, respectively. The combined testing with both ultrasonography and ^99mTc-MIBI showed that the sensitivity was elevated to 96%. Conclusions Ultrasonography and ^99mTc-MIBI are both sensitive methods for preoperative localization of parathyroid glands in patients with primary hyperparathyroidism. The combination of the two tests could improve the detection rate of abnormal parathyroid.
