血浆降钙素原测定在肺结核患者中的意义

摘　　要：目的探讨血浆降钙素原(PCT)含量测定在肺结核患者中的临床意义。方法采用半自动定量法检测26例活动性肺结核患者和24例活动性肺结核伴感染患者的血浆PCT含量,并与同期健康体检人群作对照。结果3组血浆PCT含量中位数值均在0.5ng/ml以下,用血浆PCT实际测得值进行比较,活动性肺结核组和肺结核伴感染组均明显高于健康人(P<0.01),如果以PCT0.145ng/ml作为诊断标准,则诊断肺结核与非肺结核的灵敏度为82.0%,特异度为95.0%;以PCT0.24ng/ml为标准来鉴别肺结核有无伴感染,灵敏度为79.2%,特异度为84.6%。结论血浆PCT在肺结核患者中轻微升高,可作为活动性肺结核和肺结核伴感染诊断的辅助指标。Objective assess the clinical value of serum procaleitonin （PCT） in patients with active pulmonary tuberculosis. Methods Serum procalcitonin （PCT） concentrations were measured in 26 patients of active pulmonary tuberculosis and 24 patients of pulmonary tuberculosis complicated with infection and 20 healthy subjects. Results The median procalcitonin concentrations in two pulmonary tuberculosis groups were significantly higher in comparison to the control group （P〈0.01）, but still lower than the cut-off level of 0.5 ng/ml. If taking 0.145 ng/ml as the cut-off level the specificity and sensitivity values of serum PCT in diagnosis of active pulmonary tuberculosis were 95.0% and 82.0%. If taking 0.24ng/ml as the cut-off the specificity and sensitivity values of serum PCT in diagnosis of active pulmonary tuberculosis complicated with infection were 84.6% and 79.2%. Conclusion PCT may be an useful parameter in diagnosis of pulmonary tuberculosis and pulmonary tuberculosis complicated with infection.
