吗啡控缓释片结合放疗治疗晚期肿瘤骨转移的临床疗效观察

摘　　要：目的:评价吗啡控缓释片联合放疗治疗晚期肿瘤骨转移的临床疗效和不良反应。方法:186例晚期肿瘤骨转移患者完全随机分为单纯放疗组(62例)、吗啡组(60例)和放疗联合吗啡组(联合组64例)。单纯放疗组给予局部骨转移灶单纯放疗;吗啡组按三阶梯止痛原则给予吗啡控缓释片止痛治疗;联合组在给予吗啡控缓释片止痛治疗基础上,同时局部骨转移灶进行常规放疗。结果:3组止痛疗效比较,吗啡组和联合组止痛效果明显高于单纯放疗组,P<0.05;吗啡组和联合组生活质量提高明显高于单纯放疗组,P<0.05;联合组生活质量提高明显高于吗啡组,P<0.05;联合组中位生存期明显高于吗啡组和单纯放疗组;吗啡组和联合组不良反应为恶心、呕吐、头晕、便秘。结论:吗啡控缓释片联合放疗治疗肿瘤骨转移既可达到较好止痛效果,提高生质量,延长生存期,具有较好的临床治疗效果。OBJECTIVE： To evaluate the effect and acute toxicity of morphine sulphated release tablets/hydrochloride controlled- release tablets combined with radiotherapy therapy in patients with bone metastases of terminal cancer. METHODS： 186 patients with bone metastases of terminal cancer were divided random equally into three groups： the radiotherapy group （ n = 62）, the morphine group （ n = 60） and the radiotherapy combined with morphine group （ the combined group n = 64）. the radiotherapy group treated with radiotherapy for local bone metastases, the morphine group treated with morphine sulphated release tablets/hydrochloride controlled- release tablets by three - step analgesic ladder, the combined group treated with radiotherapy for local bone metastases combined with morphine sulphated release tahlets/ hydrochloride controlled - release tablets by three - step analgesic ladder. RESULTS： Comparison of analgesic effect of the three groups, The analgesic effect of the morphine group and the combined group were significantly higher than that in the radiotherapy group （ P 〈 0.05）. The living quality of the morphine group and the combined group were significantly higher than that in the radiotherapy group （ P 〈 0.05）, The living quality of the combined group was significantly higher than that in the morphine group. The median survival time in the combined group was higher than that in morphine group and radiotherapy group. The main side effect of the morphine group and the combined group were nausea, vomiting, dizziness and constipation. CONCLUSION： There was analgesic effect of morphine sulphated release tablets/hydrochloride controlled - release tablets combined with radiotherapy therapy in patients with bone metastases of terminal cancer, prolong survival time and improve the quality＇ of life.
