摘  要:目的观察高迁移率族蛋白B1(HMGB1)在大鼠烫伤延迟复苏模型中的变化规律及其对淋巴细胞免疫功能的影响。方法96只大鼠随机分为假伤组(n=32)、烫伤组(n=32)和丙酮酸乙酯(EP)治疗组(n=32),分别于伤后1、3、5、7天活杀动物。应用Western blot方法检测血浆中HMGB1含最,采用噻唑蓝法和ELISA检测脾淋巴细胞增殖能力及培养上清中白介素-4(IL-4)和干扰素-γ(IFN-γ)浓度。结果与假伤组相比,烫伤后1、3、5天大鼠血浆HMGB1含量显著升高(P<0.05或P<0.01)。同时,脾淋巴细胞对丝裂原刺激的增殖反应在伤后1~7天明显受抑制(P<0.05);伤后1、3天脾淋巴细胞培养上清中IL-4水平明显增加(P<0.05),而伤后7天IFN-γ含量显著降低(P<0.05)。给予丙酮酸乙酯(EP)治疗组1、3、5天血浆HMGB1含量均明显下降(P<0.05),并明显减轻伤后1~7天脾淋巴细胞的增殖抑制状态,同时提高IFN-γ水平(P<0.05),而IL-4水平在伤后1、3天显著降低(P<0.05)。结论HMGB1对烫伤大鼠脾淋巴细胞免疫功能具有显著影响,它参与了烫伤延迟复苏后机体细胞免疫功能紊乱的发病过程。Objective To investigate the changes in high mobility group box 1 protein ( HMGB1 ) levels after scald injury with delayed resuscitation,and its effect on cell-mediated immunity in rats. Methods Ninty-six male Wistar rats were randomly divided into sham-scald group (n = 32) ,scald with delayed resuscitation group( n = 32) , and ethyl pyruvate treatment group( n = 32). They were sacrificed on postbum days 1,3,5 and 7 (eight rats each time point). Plasma samples were collected at each time for determination of HMGB1 levels. Spleen was harvested to determine splenic lymphocyte proliferation after in vitro cultivation with thiazolyl blue ( MTT), and concentrations Of interleukin-4 and interferon-γ in supernatant of culture medium were measured by enzyme-linked immunosorbent assay. Results Compared with sham-scald group,plasma HMGB1 levels were markedly increased on days 1 to 5 postbum (P 〈 0.05 or P 〈 0.01). The splenic lymphocyte proliferative responses to T cell mitogen were significantly.inhibited on days 1 to 7 postbum ( P 〈 0.05 ) , accompanied by an increase in interleukin-4 production on days 1 and 3(P〈0.05) and a decrease in interferon-γ on day 7 after scald injury(P〈0.05). The inhibition of HMGB1 release by ethyl pyrurate was illustrated by the suppression of proliferative response during 7-day observation period (P 〈 0.05). In addition ,treatment with ethyl pyruvate significantly decreased content of interleukin-4 release in supernatant on days 1 and 3 postbum, and in contrast the concentration of interferon-γ was dramatically increased (P 〈 0.05). Condusion These data indicated that HMGB1 can markedly influence cell-mediated immunity in scald rats with delayed resuscitation,and it might be involved in the development of postburn immune dysfunction.
